What the Miscellaneous SFSP Form Is and when it's used

The Miscellaneous SFSP Form is an administrative document used by Summer Food Service Program sponsors and subrecipients to record nonstandard requests, corrections, or program-related notes that do not fit standard USDA templates. It typically captures supplemental program information such as administrative adjustments, expense clarifications, vendor exceptions, or incident summaries submitted to state agency child nutrition offices. The form supports recordkeeping for audits, claim adjustments, and sponsor monitoring within the SFSP framework and should be retained with program files for the applicable retention period.

Why accurate completion matters for SFSP compliance

Accurate Miscellaneous SFSP Forms create an auditable trail for administrative decisions and reduce the risk of disallowed costs during USDA or state reviews. Clear records help sponsors respond to audit questions, support claim adjustments, and document corrective actions following monitoring findings.
