What the Miscellaneous Tile Form Is

The Miscellaneous Tile Form is a concise construction document used to record details about small, nonstandard tile installations, repairs, or adjustments that fall outside primary contract line items. It captures scope descriptions, material types, measurements, unit prices, and signatory approval for changes or extras on job sites. Contractors, subcontractors, project managers, and facility maintenance staff use the form to document work, establish billing or change-order eligibility, and create an audit trail for payments and inspections. When completed accurately it supports invoicing, lien waivers, and compliance with project documentation requirements.

Why a Proper Miscellaneous Tile Form Matters

Use the Miscellaneous Tile Form to reduce billing disputes, document change orders, and provide a clear on-site record of materials and labor. Accurate forms help streamline invoicing, support lien waiver processing, and create a defensible audit trail for project closeout and vendor payments.

  • Subcontractors — document materials used, quantities installed, and per-unit pricing for invoicing.
  • Owners / facilities — verify completed work, note defects, and approve final payment or retainage release.
