What the Mission Request Form for Drone Operations Is

A Mission Request Form for Drone Operations documents the who, what, when, where and safety controls for an unmanned aircraft system (UAS) activity. It gathers requester and operator contact details, aircraft and payload descriptions, proposed flight times and locations, risk mitigations, required authorizations, and any insurance or indemnity information. The form supports internal review, regulatory coordination (airspace authorizations, NOTAMs, local permits), and creates a single record for approvals, vendor contracts, incident response, and retention. It can be used on paper or submitted electronically with an audit trail.

Why a Standard Mission Request Form Matters

A standardized form reduces ambiguity, speeds review, and centralizes evidence of approvals and mitigations. Consistent submission data improves safety checks, simplifies airspace coordination, and supports insurance and compliance reviews without duplicative follow-up.

Why a Standard Mission Request Form Matters

Who Prepares and Who Reviews the Mission Request Form

The request typically originates with the project lead, site manager, or contracted UAS operator and flows to safety, legal, and permitting teams for review.

  • Project leads and site managers — initiate requests, provide operational context, and confirm site access and stakeholder notifications.
  • Designated UAS operators and pilots — supply aircraft specs, pilot credentials, and safety plans required for technical review.
  • Regulatory or permitting staff — verify airspace authorizations, local permit needs, and coordinate NOTAMs or facility clearances.

Final sign-off commonly requires authorizing manager approval and, where applicable, evidence of insurance, FAA authorizations, and any local permits before the flight.

Core Sections of a Professional Mission Request Form

A complete form groups requester details, operation specifics, safety and mitigation, authorizations, insurance, and signature blocks so reviewers can assess risk and approve consistently.

Requester

Contact name, organization, phone, and email plus billing or project code used for recordkeeping and permit tracking.

Operation Details

Date, start/end times, GPS coordinates or map, intended flight area, altitude limits, and any planned waypoint data.

Aircraft & Payload

Make/model, registration or serial number, weight class, battery type, and payload description including cameras or sensors.

Personnel

Designated remote pilot in command, visual observers, qualifications, and contact information for all operational personnel.

Risk Controls

Mitigations for people and property, line-of-sight plan, lost-link procedures, emergency landing zones, and contingency communications.

Approvals

Signatures, reviewer names, insurance limits, FAA authorizations or waivers, and any local permit references or permit numbers.

Step-by-Step: How to Complete and Submit the Form

Follow these sequential steps to prepare, review, and submit a mission request for timely processing.

  • 01
Draft the form: Enter all fields and attach supporting documents.
  • 02
Internal review: Safety and legal verify mitigations and insurance.
  • 03
Obtain authorizations: Request FAA or local permits as required.
  • 04
Final sign-off: Authorized manager signs and logs the approved mission.

Configuring an Electronic Submission Workflow

Set up routing and authentication to match your organizational approval flow and compliance requirements.

Field Configuration
Authentication Email plus SMS or stronger MFA for high-risk missions
Field Types Text, date (MM/DD/YYYY), GPS coordinate, file upload
Conditional Logic Show waiver fields only if night or beyond-line-of-sight operations
Notifications Auto-notify reviewers and record approval timestamps

Where to Send or File the Completed Form

Choose the correct internal and external recipients to ensure regulatory compliance and operational clearance.

  • Internal Safety: Safety office for risk acceptance and final clearance
  • Operations Desk: Log mission and schedule resources
  • Regulatory Filing: FAA portal or local aviation authority where required
  • Records Repository: Central archive for retention and audit trail

Options for Digital Submission and eSigning

Digital submission should include an auditable trail, signer authentication, and secure attachments.

  • File types: PDF, DOCX, JPG supported
  • Authentication: Email, SMS, or two-factor
  • Integrations: Link to cloud storage and CRM

Choose a platform that supports law-compliant eSignatures (ESIGN/UETA), secure storage, and the level of authentication your risk profile requires.

Typical Timelines and Lead Time Expectations

Plan schedules to allow internal review, permit processing, and any FAA or local coordination to be completed prior to the mission date.

Internal review lead time:

48–72 hours recommended for routine operations

Local permit processing:

Varies by jurisdiction; allow several days

FAA airspace authorization:

Authorization windows may require days to weeks

NOTAM submission:

Submit as required by facility or airspace manager

Last-minute requests:

Expect delays or denials without adequate lead time

Key Milestones from Request to Flight

Common Preparation Mistakes to Avoid

  • Incomplete site coordinates or unclear flight boundaries that lead to delayed approvals and unsafe operations.
  • Missing pilot credentials or expired certificates, which typically halt authorization and increase liability exposure.
  • Insufficient insurance detail or coverage limits that can cause permit denials or downstream claim disputes.
  • Failing to attach required FAA waivers, local permits, or landowner permissions, triggering rework and rescheduling.

Practical Examples of Mission Request Use

Two representative scenarios show how the form structures approvals and documentation for common mission types.

Site Survey Example

A construction manager submits a mission request with coordinates, equipment list, and a safety plan covering personnel exclusion zones.

  • The safety office confirms insurance and assigns a time window.
  • The recorded approval and attachments provide the project team with an auditable record for site inspections and invoice milestones, reducing scheduling conflicts and ensuring compliance with site access rules.

Media Production Example

A production lead requests aerial filming for a commercial with a detailed flight corridor and night-light plan.

  • Local permits and a noise mitigation plan are required.
  • Once authorizations, pilot credentials, and insurance are attached, the operations desk schedules the flight and notifies stakeholders, preserving the signed record for client billing and any location release requirements.
