What the Motion Request Form Is and when it’s used

A Motion Request Form is a standardized document used to ask a court or internal legal administrator to file, calendar, or process a motion in an active matter. It collects case identifiers, the movant and respondent names, the motion title and brief relief requested, proposed noticing dates, and routing instructions for filing and service. The form reduces errors by ensuring all required metadata and supporting attachments accompany a motion filing, and it clarifies whether eFiling, manual filing, or remote filing methods should be used.

Why using a Motion Request Form improves accuracy and recordkeeping

A consistent Motion Request Form standardizes the information judges, clerks, and opposing counsel need to process motions, reducing rejected filings and missed deadlines.

Who typically prepares and approves Motion Request Forms

The form supports clear accountability: who requested the motion, who prepared the package, and who completed filing or service.

  • Litigation teams and paralegals who draft and assemble motion packages for filing and service.
  • In-house legal departments requesting court filings through outside counsel or a central litigation coordinator.
  • Court clerks and docketing specialists who receive the form and complete eFiling or mail submission steps.

Primary users and signers

Paralegal

Prepares the form, attaches the motion and exhibits, and confirms eFiling requirements and service recipients. Ensures deadlines and calendaring data are correct before routing for approval.

Supervising Counsel

Reviews and authorizes the filing, signs where required, and confirms strategy (expedited hearing request, proposed order). May delegate eSubmission or instruct administrative staff to complete the filing.
