What the Moving Request Form Is and When It’s Used

A Moving Request Form documents a party’s request to relocate personnel, equipment, or household goods and captures essential logistics, dates, approvals and cost responsibilities. Typical uses include employee office relocations, tenant move notices, campus housing transfers, and vendor deliveries. The form creates a record for scheduling movers, verifying insurance coverage, assigning cost centers, and confirming access or building permissions. Accurate completion reduces delays, prevents billing disputes, and helps facilities or property teams coordinate resources, access credentials, and any required permits or service disconnections.

Why a Structured Moving Request Form Matters

A clear Moving Request Form standardizes details, assigns responsibility, and documents approvals so that operational, safety, and billing risks are reduced and coordination across teams is faster.

Why a Structured Moving Request Form Matters

Who Typically Completes or Signs a Moving Request

Organizations and individuals use the form where relocations require coordination, approvals, or cost allocation.

  • Employees requesting relocation or office moves; submits move details and preferred dates for scheduling.
  • Facilities or property managers; review logistics, building access, and vendor selection for execution.
  • HR or payroll staff; confirm relocation allowances, cost codes, and employee entitlements for reimbursements.

Clear role assignment prevents delays and ensures the right stakeholders are notified and accountable during each stage of the move.

Core Sections to Include in a Professional Moving Request Form

A complete form groups administrative details, move logistics, items to be moved, approvals, cost assignments, and liability/insurance information. Design fields to support routing, conditional approvals, and attachments so reviewers get context without follow-up.

Name, employee ID, department, phone, and preferred contact method to reach the requester for scheduling and clarifications.

Move Schedule

Requested move date and time window, lead-time requirements, and alternative dates to enable vendor scheduling and building access coordination.

Origin and Destination

Full street addresses with suite/room numbers, floor, building access notes, elevator reservations, and any restricted hours.

Inventory & Services

Detailed item list, estimated weight or counts, special handling instructions, and optional services like packing or disassembly.

Approvals & Costing

Signature lines or electronic approval fields for department head, facilities, and finance plus cost center, billing instructions, and relocation allowance details.

Insurance & Liability

Insurance coverage confirmation, mover liability limits, contact for damage claims, and required vendor certificates of insurance if applicable.

Step-by-Step: Completing and Submitting the Form

Follow this sequence to complete, route, and track a Moving Request to avoid rework and ensure timely execution.

Prepare Form: Enter requester and move details completely.
  • 02
Attach Inventory: Upload or paste item lists and photos.
  • 03
Obtain Approvals: Route to department and facilities approvers.
  • 04

Submit at least 14–30 days before requested move.

Approval Window:

Approvers should respond within 3–7 business days.

Vendor Scheduling:

Finalize vendor booking 7–14 days before move.

Move Confirmation:

Common Mistakes to Avoid When Preparing the Form

  • Incomplete address details cause delayed access or missed deliveries; include suite, floor, entry code, and loading-dock instructions to prevent no-entry events.
  • Unclear inventory descriptions lead to disputed damage claims and missing items; use itemized lists with photos for high-value or fragile goods.
  • Missing approvals or incorrect cost center codes delay scheduling and can shift billing responsibility; verify approver names and finance codes before submission.
  • Neglecting access or parking permits may incur fines or rescheduling fees; reserve elevator times and loading zones when required by building management.

Risks and Potential Consequences of Errors

Scheduling Delays: Additional vendor fees
Denied Access: Move cannot occur
Billing Disputes: Incorrect cost allocation
Damage Claims: Liability disputes
Regulatory Fines: If permits are missing
Data Exposure: Personal data mishandling risk
