What the Moving Services Move Options document is

The Moving Services Move Options document is a standardized selection and service summary used by movers and customers to record chosen move services, valuation coverage, accessorials, pickup and delivery windows, itemized pricing, and signature consent. It functions as an addendum to a master moving contract or as a standalone worksheet used during booking, survey, or estimate confirmation. The form clarifies responsibilities, documents optional services (packing, storage, appliance disconnect), and creates an auditable record suitable for electronic signing under U.S. e-signature laws when all required consent and retention steps are met.

Why a clear Move Options form matters

A complete Move Options document reduces misunderstandings, speeds billing, and supports claims handling by documenting exactly which services and protections were selected.

Why a clear Move Options form matters

Who commonly completes or receives this form

Movings Services Move Options is used by both service providers and customers during quoting, booking, and delivery.

  • Moving company operations and estimates teams confirm services and charges before dispatch.
  • Household or business customers select coverage level, packing, and timing preferences.
  • Property managers or relocation coordinators verify move windows and building rules.

The completed form serves as a mutual record for operational coordination, invoicing, and dispute resolution.

Primary signers and their roles

Operations Manager

The operations manager or dispatcher approves available services, confirms crew assignments and timing, and signs to authorize carrier obligations. Their signature confirms the carrier’s scope of work and any accessorial pricing agreed at time of booking; retain for operational audit and claims support.

Customer / Shipper

The customer or shipper selects move options, acknowledges valuation/insurance terms, and signs to confirm consent. Their signature documents acceptance of charges, chosen protection level, and delivery instructions; mismatches between signed options and actual services complicate claims or dispute resolution.

Key data elements recorded on the form

Full legal name: As on government ID
Service address: Street, city, state, ZIP
Effective date: MM/DD/YYYY
Valuation selected: Declared value or coverage type
Accessorials chosen: Packing, stairs, elevator, storage
Signature and timestamp: Signer, method, time

Consequences of incomplete or incorrect entries

Claim denial: Incorrect valuation details
Service delays: Missing accessorials
Billing disputes: Unclear pricing or dates
Void coverage: Unsigned acknowledgements
Legal disputes: Conflicting contract terms
Regulatory risk: Failure to retain records

Common preparation mistakes to avoid

  • Leaving valuation or protection fields blank — without declared coverage the default carrier liability often applies and may be minimal compared with replacement value.
  • Using vague service descriptions like 'packing included' without specifying full or partial packing and number of crew hours, which causes disputes on arrival and billing.
  • Failing to record building access restrictions, elevator reservations, or required certificates leading to delays and additional accessorial charges assessed after the move.
  • Not capturing delivery contact details or alternate recipient authorization, resulting in failed deliveries and potential storage or re-delivery fees.
