1. APPLICATION FOR RENEWAL

Applicant submits this application to renew the notary commission identified above. Applicant certifies under penalty of perjury that the information provided on this form is true and complete to the best of Applicant's knowledge and that Applicant remains qualified for commission under applicable law.

2. OATH, BOND AND INSURANCE

As a condition of renewal, Applicant shall, if required by statute, execute and file an official oath and procure and maintain a surety bond in the amount required by law. Applicant affirms that any bond submitted in connection with this renewal application meets statutory requirements and will be effective on the date of commission renewal.

3. CERTIFICATION OF QUALIFICATIONS

Applicant certifies that Applicant is at least eighteen (18) years of age, is a legal resident of the Commissioning State, is not disqualified from holding office under law, and has not been convicted of a felony or any offense involving fraud or dishonesty unless civil rights have been restored.

4. FEES

5. DUTIES, CONDUCT AND RECORDS

Applicant acknowledges the duty to perform notarial acts in accordance with applicable law, including maintaining an official journal of notarial acts where required, using an official seal as prescribed by statute, and safeguarding the integrity of all notarial records. Applicant agrees not to engage in the unauthorized practice of law while performing notarial acts.

6. RECORDS OF NOTARIAL ACTS

Where law requires, Applicant shall keep a journal of notarial acts containing at minimum the date and time of each notarial act, type of act, name and address of each principal, evidence of identity, fee charged, and a brief description of the document notarized. Such journal shall be produced to the Authority upon lawful request.

7. NOTICES

Any notice required or permitted under this Agreement shall be given in writing and delivered to the addresses below via registered mail or other reliable delivery service and shall be deemed given upon receipt.

8. AMENDMENTS, WAIVER, GOVERNING LAW

This Agreement may be amended only by a written instrument signed by both parties. No provision of this Agreement may be waived except by a written waiver signed by the party against whom enforcement of the waiver is sought. This Agreement shall be governed by and construed in accordance with the laws of the State of without regard to conflict of laws principles.

9. ENTIRE AGREEMENT AND SEVERABILITY

This document constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior understandings, agreements, or representations. If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.

10. ACKNOWLEDGMENT AND CERTIFICATION
