What a Notary Request Form Is and when it’s used

A Notary Request Form is a standardized document used to collect the information a notary public needs to complete a notarization request. It records requester and signer details, the document(s) to be notarized, the type of notarization required (in-person or remote online notarization), identification presented, and any witness requirements. Organizations use this form to reduce processing delays, provide clear instructions to the notary, and create a verifiable record of the notarization request and related attachments for retention and audit purposes.

Why a clear Notary Request Form matters

A concise, accurate Notary Request Form minimizes procedural errors, speeds notarization, and supports legal enforceability by documenting intent, identity verification, and method of notarization.

Why a clear Notary Request Form matters

Who typically completes and relies on this form

The Notary Request Form is completed by parties coordinating notarizations and by administrative staff who schedule or document the process.

  • Real estate agents and title officers who need notarized deeds and closing documents.
  • Legal and corporate counsel arranging notarization for contracts, affidavits, or powers of attorney.
  • Healthcare and HR administrators managing consent, release, or employment documents.

Accurate completion by these roles helps ensure valid notarizations and consistent retention of the notarization record.

Step-by-step: completing a Notary Request Form

Follow these steps to gather the details a notary needs and to avoid delays when scheduling or executing the notarization.

  • 01
    1. Collect basic data: Enter requester and signer full legal names exactly as shown on ID.
  • 02
    2. Identify the document: List each document title and number of pages to be notarized.
  • 03
    3. Choose notarization type: Specify in-person or RON and any witness requirements.
  • 04
    4. Provide appointment details: Include preferred dates/times and contact information for scheduling.

Where to send or submit the completed form

Choose the correct recipient so the notary or scheduling team can act without delay; include contact and routing instructions.

  • To the Notary: Send completed form plus IDs and documents for pre-review.
  • County Recorder: Forward final notarized deed or instrument for recording if applicable.
  • Legal Counsel: Share copies when review or signature confirmation is required.
  • Document Custodian: Store the executed original and any recordings in the designated repository.

Technical considerations for eSubmission and RON

Remote notarizations and digital submissions can require specific formats, integrations, and authentication steps.

  • File formats: Accept common formats: PDF, DOCX; PDF/A recommended for long-term storage.
  • Authentication: RON often requires multi-factor identity proofing and recorded audio-video.
  • Integrations: Integrate with systems like Salesforce, NetSuite, or Google Workspace for automated routing.

Confirm platform capabilities for recording retention, tamper-evidence, and audit logs before scheduling RON sessions.

Essential components of a professional Notary Request Form

A complete form organizes the information a notary and recipient need to verify identity, confirm intent, and process the notarization efficiently.

Requester details

Name, organization, contact phone and email for scheduling and follow-up; crucial for invoicing and tracking chain of custody.

Signer identity

Full legal name, date of birth, and acceptable ID types to reduce identification issues at signing.

Document inventory

Exact titles, page counts, and any exhibits or annexes so the notary can confirm completeness before execution.

Notary instructions

Specify type of acknowledgment, jurat, or witnessing, plus any state-specific witness count or witnesses' details.

Scheduling details

Preferred times, time zone, physical or virtual meeting link, and contingency instructions for no-shows.

Attachments checklist

List of IDs, prior documents, and signed declarations to be provided at the session for verification and recordkeeping.

How to save and distribute executed documents

Preserve notarized records in formats and locations that support long-term access and legal admissibility.

Certified PDF (PDF/A)

Export notarized documents as PDF/A with embedded signature metadata to support long-term archiving and reduce compatibility issues.

Standard PDF

Provide each party a flattened signed PDF for routine exchange and immediate review after notarization.

Word DOCX

Keep source DOCX copies for editable templates but do not treat them as legal originals after signing and notarization.

Supplemental files

Attach audio/video RON recordings, identification images, and the notary journal entry when retaining the complete execution record.

Timing considerations and common deadlines for requests

Plan lead times and retention expectations to avoid scheduling conflicts or compliance lapses.

Scheduling lead time:

Request notarization at least 3 business days before the preferred date.

RON recording retention:

Many states require audio/video retention for 5–10 years for RON sessions.

County recording:

Record deeds promptly; county deadlines and indexing vary by jurisdiction.

I-9 document timing:

Employers must retain I-9 according to 8 CFR §274a.2 (3 years after hire or 1 year after termination).

Document revisions:

Allow time for corrections; corrected notarizations typically require re-execution and fresh notarization.

Consequences of incomplete or incorrect notarization requests

Invalid notarization: Record may be rejected or unenforceable
Perjury exposure: False statements can trigger criminal liability
Recording rejection: County recorder may refuse defective deeds
Tax penalties: Missing TINs can trigger 24% backup withholding
Notary liability: Notary bond or disciplinary actions for improper acts
RON failures: Missing recordings can void remote notarizations

Common preparation errors to avoid

  • Entering abbreviated names that do not match government IDs, which can require re-signing and delay recording.
  • Failing to include the correct document title or page count, causing the notary to withhold notarization until review.
  • Selecting RON in a jurisdiction that lacks permanent RON rules, resulting in rejected sessions or rework.
  • Omitting witness information where a state requires two witnesses, which can invalidate certain affidavits or deeds.

Practical tips for accurate and efficient requests

Adopt standard templates and verification steps to reduce rework and support consistent compliance across notarizations.

Use exact legal names
Require the signer to provide the name as it appears on government ID; confirm spelling and suffixes (Jr., III) to avoid mismatches and re-execution.
Confirm notarization type
Verify whether in-person notarization or RON is legally permitted for the document and jurisdiction before scheduling a remote session.
Attach required IDs
List acceptable ID types and request copies in advance to speed the session and reduce identity-verification friction.
Keep complete records
Retain the form, executed document, audit trail, and any RON recordings in secure storage for the period required by law.

Security and compliance considerations
