What a Nursing Services Addendum Is and When It Applies

A Nursing Services Addendum is a formal amendment or attachment to an existing service, provider, or facilities agreement that defines the scope, responsibilities, and terms for nursing or medical support services. Typical uses include contracting school nurses, facility-based clinical staffing, home health addenda, or supplemental staffing arrangements that clarify scope of practice, supervision, liability allocation, credentialing requirements, and applicable rates. The addendum is used alongside a master services agreement or vendor contract and can address insurance, HIPAA protections, background checks, licensing verification, and termination terms specific to nursing services.

Why the Nursing Services Addendum Matters

A clear addendum reduces ambiguity about clinical duties, licensure expectations, and recordkeeping, lowering regulatory risk and disputes.

Why the Nursing Services Addendum Matters

Who Typically Prepares and Signs a Nursing Services Addendum

Organizations and individuals involved in clinical support agreements should prepare or review the addendum before services begin.

  • Healthcare providers and clinics who subcontract nursing time for clinics, schools, or home health.
  • School districts or educational institutions arranging on-site nursing support for students.
  • Long-term care and assisted living operators adding temporary or supplemental nursing services.

Final signing should include authorized representatives from both parties plus any required institutional approvers or compliance officers.

Who Signs and Their Roles

Director of Nursing

A Director of Nursing or equivalent clinical leader typically certifies clinical scope, staffing ratios, and credential verification processes; they confirm clinical protocol alignment and escalation paths in 2–3 concise paragraphs.

Authorized Official

A facility administrator or procurement officer signs for the recipient organization to accept service terms, billing rates, insurance requirements, and termination rights while ensuring the addendum aligns with institutional policies.

Essential Sections Found in a Professional Nursing Services Addendum

A standard addendum organizes terms so clinical, legal, and administrative teams can quickly find responsibilities, credentials required, and who is liable for what.

Scope of Services

Defines tasks (e.g., assessments, medication administration), hours of coverage, on-call duties, and exclusion of services not authorized under the contract.

Credentials & Background

Specifies required licensure, certifications, TB/health screenings, criminal background checks, and proof of malpractice coverage limits.

HIPAA & Privacy

Lists HIPAA compliance obligations, whether a BAA is required, permitted PHI uses, and breach-notification procedures.

Indemnity & Insurance

States liability allocation, minimum insurance limits, and requirements for naming the contracting party as an additional insured.

Billing & Rates

Explains hourly rates, billing intervals, overtime, reimbursement rules, invoicing cycle, and any allowable expenses.

Termination & Transition

Describes notice periods, wind-down obligations, patient record transfer, and continuity plans for ongoing care.

Contractual Liability: Breach damages and indemnity obligations may accrue.
Licensing Risk: Unlicensed practice can trigger administrative penalties.
Payment Delays: Incorrect payee or tax info can cause withholding.
Service Disruption: Lack of transition terms may interrupt patient care.
Notarization Errors: Missing notarization or witnesses where required can void execution.
