What the Occupational Health Assessment Form Is

The Occupational Health Assessment Form documents a worker's medical status, exposure history, and fitness-for-duty determinations used by employers, occupational health providers, and safety teams. It typically captures identifying information, job duties, relevant medical history, immunizations, functional limitations, required work restrictions, and the clinician's conclusions. The completed form supports return-to-work decisions, exposure follow-up, accommodation assessments, and regulatory recordkeeping while distinguishing workplace medical records from routine HR files.

Why this form matters for workplace health and compliance

Accurate occupational health assessments protect employee safety, inform reasonable accommodations, and create an auditable record for compliance with OSHA, HIPAA, and other workplace rules.

  • Using inconsistent date formats or leaving provider credentials out of the signature block complicates record validation and auditability.
  • Storing completed assessments in general HR files rather than in medical records can expose PHI to unauthorized access.
