What the Office Discovery Document Is and when it's used

An Office Discovery Document is a standardized intake and assessment form used to capture essential information about a physical office, its contacts, systems, and operational constraints. Typical uses include new-site onboarding, facilities audits, IT network provisioning, vendor readiness checks, lease or occupancy reviews, and relocation planning. The document collects contact names, emergency contacts, floor plans, network and power details, security access, equipment inventories, and any special compliance or accessibility notes needed to plan services or contractual work at the location.

Why a formal Office Discovery Document matters

A consistent Office Discovery Document reduces misunderstandings, speeds technical and facilities preparations, and creates a single source of record for scope, compliance needs, and risk assessment. It also documents decisions and baseline conditions that reduce disputes during onboarding or change management.

Why a formal Office Discovery Document matters

Who typically completes and relies on this document

Teams across operations, IT, real estate, and vendor management complete or consult the Office Discovery Document to coordinate a new engagement or assess site readiness.

  • Facilities or office managers who supply floor plans, access procedures, and on-site contacts for contractors and vendors.
  • IT or network administrators who provide network diagrams, VLAN details, Wi-Fi SSIDs, and device inventory for provisioning.
  • Project managers and procurement teams who use the record for scope validation, scheduling, and invoicing reconciliation.

Capturing these roles in one document shortens setup cycles and provides a defensible record of pre-existing conditions.

Representative signatories and approvers

Office Manager

An Office Manager or Site Lead signs or certifies the accuracy of site-specific information (floor plans, access hours, emergency contacts). This person typically coordinates vendor arrivals and is authorized to confirm operational details for short-term engagements.

IT Lead

An IT Lead or Network Administrator attests to network and system details, such as IP ranges and device lists. Their signature confirms that proposed technical work aligns with existing configurations and security policies.

Core sections to include in a professional Office Discovery Document

A complete document groups information so reviewers can find operational, security, and compliance details quickly; use clear section headings and consistent field formats.

Site Identity

Legal site name, full street address, suite number, facility type, and primary contact. Include alternative access directions and business hours for accurate scheduling and deliveries.

Contacts

List primary and backup contacts with titles, phone numbers, email addresses, and on-site hours. Identify the person authorized to approve vendor access or contract amendments.

Infrastructure

Power capacity, breaker locations, HVAC constraints, server room details, and on-site UPS or generator information needed for equipment installation and safety planning.

Network & Systems

Network topology, public IPs, Wi-Fi SSIDs, VLANs, port maps, and remote access methods. Note maintenance windows and restrictions on changes during production hours.

Security & Access

Access control systems, badge procedures, visitor policies, CCTV presence, and any credential or escort requirements for external personnel entering sensitive areas.

Compliance Notes

Any industry-specific requirements such as HIPAA controls, FERPA data handling, or physical security standards. Flag areas requiring BAAs or privacy addenda before work begins.

Step-by-step: completing the Office Discovery Document

Follow this order to collect accurate information and minimize rework.

  • 01
Gather contacts: Collect all site and escalation contacts.
  • 02
Capture infrastructure: Record power, HVAC, and room constraints.
  • 03
Document network: List IPs, VLANs, and Wi-Fi details.
  • 04
Review and sign: Authorized parties verify and sign the form.

Typical document flow from completion to operational use

A clear routing sequence ensures each stakeholder receives required sections and approvals before work begins.

  • Create: Originator completes initial site and contact information.
  • Validate: IT and facilities review technical and safety details.
  • Approve: Authorized signatories confirm readiness and constraints.
  • Distribute: Finalized document is shared with vendors and retained in records.

How to configure a digital workflow for the Office Discovery Document

Configure fields and routing to match internal approval steps and ensure the record is retained in a central system.

Field Configuration
Site Identity Required; text field; validate address format
Signature Required; signer order; capture timestamp
Attachments Enable PDF/DOCX upload for floor plans
Retention Tag Assign retention class for records management

Technical and compliance requirements for digital completion

Ensure the signing platform supports audit trails, acceptable authentication, and secure storage to meet legal and policy requirements.

  • Authentication: Email link or SMS code are common; use stronger authentication for sensitive sites.
  • File formats: Support for PDF and DOCX preserves layout and attachments.
  • Integrations: Connect to your document repository or ticketing system for automated retention and access.

Platforms should produce an immutable audit trail and exportable signed records to satisfy ESIGN/UETA record retention requirements.

Essential secure data fields to collect

Emergency Contact: Name and phone
Access Hours: Site open times
Server Room: Location and restrictions
Badge Policy: Badge required/escort
HIPAA Flag: PHI present/absent
Data Classification: Public/internal/confidential

Common mistakes to avoid when preparing the document

  • Submitting incomplete network ranges or missing VLANs, which causes configuration errors and delays for provisioning teams.
  • Using inconsistent contact details or outdated phone numbers that prevent timely coordination for vendor arrivals and escalations.
  • Failing to flag compliance constraints (for example HIPAA or FERPA) that require contract addenda or special access controls.
  • Attaching low-resolution floor plans or failing to mark critical clearances, producing unsafe site setups or rework on installation days.

Risks and compliance consequences of incorrect or missing information

Operational Delay: Missed deadlines
Safety Incident: Unidentified hazards
Contract Disputes: Scope and cost disagreements
Regulatory Exposure: Noncompliance fines
Privacy Breach: HIPAA violations
Financial Loss: Remediation and rework costs
