What the OSER Waiver Form Is and when it applies

The OSER Waiver Form is a standardized document used to request an administrative waiver or exception from requirements managed by an OSER-designated agency or office. It records the applicant's identity, the specific regulatory requirement being waived, factual justification, supporting documentation, and the requested effective period. Although the exact name and fields vary by agency, the waiver form generally creates a written record of consent, authority review, and final disposition. Proper completion preserves procedural rights and speeds agency processing while creating an auditable trail for compliance purposes.

Why the OSER Waiver Form matters for applicants and administrators

A clear OSER Waiver Form aligns the applicant’s request with agency criteria, reduces back-and-forth, and documents the legal basis for any exception. For administrators, a consistent form simplifies review, ensures required evidence is attached, and supports defensible decisions.
