What the OSHA Safety Questionnaire Is and When it’s Used

The OSHA Safety Questionnaire is a structured form used to collect workplace safety information from employers, contractors, or site supervisors to support compliance with OSHA recordkeeping, hazard communication, and site-specific safety programs. It typically documents workplace hazards, personal protective equipment, training status, incident history, and exposure controls. Organizations use the questionnaire to prepare OSHA 300/301 logs, evaluate site risk, satisfy internal audits, and demonstrate due diligence during inspections. Responses should be factual, contemporaneous, and retained according to applicable federal and state recordkeeping rules.

Why a Clear OSHA Safety Questionnaire Matters

A complete questionnaire helps identify hazards, supports accurate OSHA injury and illness records, and demonstrates compliance with 29 CFR Part 1904 and other safety regulations. It reduces inspection risk and improves corrective action targeting.

Why a Clear OSHA Safety Questionnaire Matters

Which Roles Typically Complete the Questionnaire

Ensure the person completing the form has current knowledge of site conditions and access to training and incident records to avoid incomplete or inaccurate responses.

  • Safety managers and EHS professionals responsible for site hazard assessments and OSHA recordkeeping.
  • Supervisors or forepersons on active job sites documenting incidents, PPE usage, and training status.
  • Contract administrators or procurement staff assessing subcontractor safety controls before site access.

Core Sections to Include in a Professional OSHA Safety Questionnaire

A practical questionnaire is concise but thorough: include identification, hazard inventory, training verification, incident history, exposure controls, and follow-up actions. Each section should allow dates, checkboxes, and brief narrative answers.

Site Identification

Name, address, employer, job site contact, and date — essential for linking responses to the correct OSHA log.

Hazard Inventory

List observed physical, chemical, biological, ergonomic, and electrical hazards with location and frequency notes.

Training Status

Record required safety training completed (course name, date, trainer) and gaps that require remediation.

Incident History

Document recordable injuries, near-misses, first-aid events, and dates to inform OSHA 300/301 entries.

Controls & PPE

Describe engineering, administrative controls, and PPE provided or required for tasks and exposures.

Corrective Actions

Capture recommended fixes, responsible party, and target completion date for closure tracking.

Required Data Elements to Capture

Employee Name: Full name
Job Title: Role or trade
Date of Event: MM/DD/YYYY
Location: Worksite address
Injury Type: Nature of injury
Corrective Action: Remedy assigned

Step-by-Step: Completing the OSHA Safety Questionnaire

Follow a consistent sequence to collect accurate data and reduce follow-up queries.

  • 01
    1. Identify: Confirm site, employer, and date before gathering details.
  • 02
    2. Observe: Walk the site, note hazards, and verify controls in place.
  • 03
    3. Record: Enter incident facts, witness names, and injury descriptions.
  • 04
    4. Assign: Document corrective actions, responsible person, and due dates.

Typical Workflow from Questionnaire to Recordkeeping

The questionnaire feeds internal review, injury classification, and, if required, OSHA log entry and corrective action tracking.

  • Collect Data: Complete questionnaire at time of observation or immediately after incident.
  • Review: EHS or supervisor reviews entries for clarity and completeness.
  • Classify: Determine recordability per 29 CFR Part 1904 criteria.
  • File & Track: Retain questionnaire with supporting documentation and track corrective actions.

Configuring a Digital Questionnaire Workflow

Set up fields, routing, and access controls so responses are captured, reviewed, and archived consistently.

Field Configuration
Required Fields Mark site, date, and reporter as mandatory
Reviewer Role Assign EHS reviewer for approval routing
Notifications Auto-notify responsible persons for corrective actions
Archive Store in secure, access-controlled repository

Technical Considerations for Digital Submission and eSigning

Ensure the chosen solution provides tamper-evident records, audit logs with timestamps, and access controls to support OSHA investigations and internal compliance reviews.

  • Document Formats: PDF, DOCX, and HTML supported for form import/export
  • Integrations: Connectors to HR/ERP systems (e.g., HRIS, NetSuite, Salesforce)
  • Authentication: Options: email link, SMS code, or advanced signer verification

Timelines and Recordkeeping Expectations

Timely completion and retention of safety questionnaires underpin OSHA recordkeeping and support regulatory responses when inspections occur.

Immediate Completion:

Document incidents and observations as soon as possible to preserve accuracy.

OSHA Log Entry:

Recordable injuries must be entered on OSHA 300/301 per criteria during the calendar year they occur.

Retention Minimum:

Retain injury-related records for 5 years after the year of the record (29 CFR §1904.33).

Long-Term Exposure Records:

Maintain exposure and medical records per applicable rules (e.g., 29 CFR §1910.1020 for OSHA exposure).

Internal Review Cadence:

Review questionnaires and corrective actions at least quarterly for active sites.

Common Pitfalls to Avoid

  • Delaying completion — time gaps increase factual uncertainty and weaken record credibility.
  • Incomplete contact info — missing reporter or witness names complicate follow-up and verification.
  • Vague descriptions — lack of specifics can cause misclassification under OSHA recordability rules.
  • Not tracking closures — open corrective actions undermine auditability and repeat hazards.

Risks and Penalties from Inaccurate or Missing Records

OSHA Citations: Fines and notices can follow incomplete recordkeeping or failure to maintain required logs.
Inspection Delays: Missing documentation prolongs inspections and may increase enforcement scrutiny.
Civil Liability: Incomplete records can weaken employer defenses in litigation or compensation claims.
Reputational Harm: Repeated recordkeeping failures can affect contracting eligibility and public trust.
Operational Impact: Unaddressed hazards increase injury risk and lost-time incidents.
Regulatory Escalation: Chronic noncompliance may trigger higher penalties and programmatic oversight.
