What an Owner Information Request Is and When It’s Used

An Owner Information Request is a standardized form used to collect verifiable ownership and contact details for individuals or entities that hold legal or beneficial interest in property, accounts, or business relationships. Typical uses include onboarding property owners, verifying payee information for tax reporting, capturing authorized signatory details for corporate records, and documenting chain-of-title facts. The form focuses on identity, ownership percentage, tax identification, mailing and service addresses, and authorization signatures to support legal, financial, and regulatory processes in the United States.

Why a Complete Owner Information Request Matters

Accurate owner information reduces compliance risk, enables correct tax reporting, and supports clear title and authorization chains. Proper documentation helps avoid backup withholding, misdirected notices, and later disputes under ESIGN (15 U.S.C. ch. 96) and state electronic transaction laws (UETA).

Why a Complete Owner Information Request Matters

Who Typically Prepares and Receives This Form

Organizations and individuals that need verified ownership or payee details complete or request this form during onboarding and transactions.

  • Real estate brokers and property managers onboarding owners or investors for leases and closings.
  • Financial institutions and lenders verifying beneficial owners and tax information for accounts.
  • Title companies, escrow agents, and legal counsel documenting ownership for transfers and closings.

Accurate completion keeps downstream processes—tax reporting, title transfer, claims handling—moving without delay.

Core Elements to Include in a Professional Owner Information Request

A clear, professional Owner Information Request groups identity, ownership, tax, contact, authorization, and evidence fields to support legal and tax obligations while enabling reliable electronic completion and retention.

Owner Identity

Full legal name, business entity name, and any DBAs to match government IDs and tax records; avoids name-mismatch issues.

Ownership Details

Ownership percentage, class of interest, and effective date of ownership to clarify reporting obligations and voting/consent rights.

Tax Information

Taxpayer Identification Number (TIN/EIN), W-9 or equivalent request language, and backup-withholding status to support IRS reporting.

Contact and Service Address

Physical address, mailing address, email, and phone for notices, tax forms, and legal service; P.O. boxes alone may be insufficient for service.

Authorization

Statement of authority for signers, title or role, and date of appointment to prevent unauthorized agreements.

Evidence Attachments

Attach government ID, corporate formation documents, or EIN confirmation to substantiate identity and authority.

Common Mistakes to Avoid When Preparing the Form

  • Entering trade names or nicknames instead of the legal owner name, which causes mismatches with tax or title records and triggers verification delays.
  • Omitting the TIN/EIN or providing an incorrect number, which can lead to backup withholding or information return rejections by the IRS.
  • Attaching low-quality scans of IDs or formation documents that fail automated verification and require manual review or re-submission.
  • Using P.O. boxes when a physical address is required for legal service or tax notices, creating delivery and compliance problems.

Penalties and Operational Risks of Incomplete or Incorrect Data

Backup Withholding: 24% withholding may apply
Tax Filing Penalties: IRC §6721 penalties per incorrect return
Delayed Transactions: Closings and payouts may be postponed
Reputational Risk: Client disputes and trust erosion
Compliance Violation: HIPAA or privacy breaches increase liability
