Release, Photo & Medical Authorization

Contestant assumes the risk of participation and agrees to comply with safety rules.

Photo/Media Release: Contestant grants Organizers the perpetual, royalty-free right to photograph, record, reproduce, and distribute Contestant's image, voice, and performance for promotional and archival purposes in any medium.

Medical Authorization: In the event of illness or injury, Organizers are authorized to obtain emergency medical treatment for the Contestant when a Parent/Guardian cannot be reached. Contestant (or Parent/Guardian) shall disclose any medical conditions or allergies in the field below.
