Loss of Funding: Forfeiture of fundraising proceeds
FERPA Violations: Disciplinary action and remediation
Volunteer Restrictions: Loss of volunteer privileges
Tax Penalties: IRS penalties for poor recordkeeping
Event Cancellation: Insufficient permissions cancel activities

Practical Tips for Accurate, Efficient Completion

Use a consistent process for drafting, approval, and storage to minimize errors and ensure compliance with district and federal rules.

Standardize consent language and templates across events
Create approved templates for field trips, media release, and medical authorizations. Include clear scope, expiration dates, emergency contact sections, and a withdrawal process. Periodically review templates with district counsel to remain consistent with FERPA and local policies.
Keep clear financial records and receipts
Track all fundraising income, itemize expenses, obtain receipts for purchases, and require two signatories on large disbursements. Maintain monthly reconciliations and annual financial reviews to satisfy IRS and district expectations for nonprofit organizations.
Verify signer identity and consent methods
Confirm signers' emails and phone numbers, use two-factor authentication where available, capture audit trails with IP and timestamps, and save consent disclosure records for consumer-facing permissions per ESIGN requirements and retention policies.
Review state and district rules annually
Check state notary, witness, and RON rules each year and confirm district policies on student privacy, volunteer background checks, and financial filings. Document any changes and update templates promptly to maintain compliance.
