Parental Consent and Release Form Bus Travel to Hockey Games and Practices Agreement made on the _______________________ (date) , between ____________________________ (Name of Parent or Guardian) , of (street address, city, county, state, zip code) , referred to herein as Parent/Guardian , and _____________________ _____________________ (Name of Hockey Association) , of ____________________________________________________________________________ (street address, city, county, state, zip code) , referred to herein as Association . I. The undersigned Parent/Guardian of __________________________ (Name of Player) understands and agrees that my child’s participation in the game of Hockey organized and managed by Association is a privilege, and not a right. I acknowledge that I have spoken with my child about my child’s need to comply with the specific rules and requirements established for this activity, all of the Association’s rules, policies and procedures, and specifically the rules of conduct set forth below regarding bus travel to and from games and practices. I understand that all Association rules and policies apply to my child, practically the rules and policies regarding bus travel to and from games and practices. I also understand that I have the ability to refuse to sign this Form, and that if I refuse to sign, my child will not be permitted to participate in the hockey activities sponsored by the Association. II. By signing this form, I hereby release the Association, as well as its directors, officers, administrators, employees, volunteers, and other agents from all liability for any and all injuries arising out of my child’s participation in these hockey activities, particularly regarding bus travel to and from hockey games and practices. I further agree to indemnify and hold harmless the Association , as well as its directors, officers, administrators, employees, and other agents, against any claims asserted by my child or myself as a result of his or her participation in these activities and bus travel. III. I acknowledge that my child’s failure to comply with such rules and requirements may result in discipline, up to and including suspension or expulsion from the team and hockey activities of the Association. I also understand that I have the ability to refuse to sign this Consent Form, and that if I refuse to sign, my child _____________________ (Name of Player) will not be permitted to participate in these hockey activities. IV. I give permission for my child to be transported to and from hockey games and practices by bus in [describe area, e.g., in the metropolitan area of (name of city)] ______________ ____________________________________________________________________________. I have read and discussed this form with my child and grant my approval for him/her to participate in hockey activities of the Association according to these requirements. My child has read and has agreed to abide by these rules, regulations and guidelines set forth by the Association. Rules and Regulations regarding Bus Travel to Hockey Games and Practices 1. Appropriate bus behavior is expected of all hockey athletes. 2. All players must go to and return from hockey practices and games by Association supplied vehicles. If a player wishes to return with his/her parents, he/she must have the parent make the request in writing to the coach prior to the game or practice. 3. Players shall be under the supervision of the coach and bus driver. 4. Bus rules will be followed on all bus trips. 5. Avoid horseplay. 6. Respect other people's property. 7. Wait in an orderly manner while getting on and off the bus. 8. One person at a time will get on and off the bus. 9. Use the handrail. 10. Find a seat quickly, and share seats with others when necessary. 11. Keep feet, legs and personal belongings out of aisle. 12. Remain seated, facing forward, at all times while the bus is in motion. 13. Heads and hands must be kept inside the bus at all times. 14. Avoid horseplay. 15. Players are expected to conduct themselves in an appropriate manner while riding on buses. 16. The bus driver has full and final authority over all passengers while they are on the bus. All players shall follow the directions given by the driver. 17. All players shall remain seated while the bus is in motion. 18. Players are expected to sit properly in their seats (facing forward), refrain from throwing objects on the bus, refrain from disturbing other players, refrain from creating excessive noise that distracts the driver, and assist in keeping the bus clean by keeping their waste paper and other trash off the floor. 19. Players must arrive at the bus on time (5 minutes before the scheduled bus arrival time) and should not stand or play on the roadway while waiting for the bus to arrive. 20. Self discipline should be exercised by players at the bus loading area. 21. Injury causing items may not be transported on the bus. Such prohibited items include, but are not limited to the following: sticks, breakable containers, any type of firearm, laser pointers, pins extending from clothing, aerosol containers clothes hangers, etc. 22. Smoking, lighting matches, or use of any type of flame or sparking devices is prohibited. on the bus. 23. At no time shall players extend their head, hands, or arms out of the windows whether the school bus is in motion or standing still. 24. No refuse shall be thrown out the windows. 25. Players shall refrain from using vulgar or obscene language or gestures on the bus. 26. All personal belongings will be kept out of the aisle. Large bulky items which will block the aisle or exits may not be transported on the bus. 27. Passengers should refrain from talking to the bus driver while the bus is in motion. 28. No animals may be transported on the bus. 29. Players shall be familiar with emergency evacuation procedures. In the event of an actual emergency, players must follow proper emergency evacuation procedures. 30. The Association shall be reimbursed (by the player’s parent or guardian for damage to buses resulting from the misconduct of the players involved. 31. Place trash in the proper place. 32. No loud or disruptive behavior will be tolerated. 33. Do not throw objects inside or outside the bus. 34. Do not leave items on the bus at the end of the trip. 35. In case of emergency, remain on the bus until the driver instructs you otherwise. 36. Any conduct deemed by the coach or driver to be detrimental may result in disciplinary actions. These actions may include but are not limited to suspension or removal from a team. 37. Disciplinary action will be determined depending on the severity of the situation. 38. H azing of any kind and physical or verbal harassment is not condoned nor acceptable. 39. Talk quietly with no inappropriate language or gestures. Quietly means at a level where the driver cannot clearly hear the conversation and the player you are trying to talk to are no more than two seats away. 40. Music devices may be used while on the bus, but must be used with headphones. 41. Bus windows will not be lowered more than half the distance of their travel. 42. Cell phones and cameras used during bus travel will be confiscated and disciplinary action may be taken. 43. No items will be stored in the back window. 44. Players must dress neatly, appropriately and in good taste, while traveling on the bus. 45. Players must wear shoes (slippers, sandals, etc. are not permitted) 46. The emergency door at the rear of the bus should not be touched unless a real emergency exists or the driver gives specific instructions. 47. Players shall not exhibit behaviour that will distract the driver or prevent him/her from operating the bus safely . WITNESS our signatures on ___________________ (date) .
