I, the undersigned parent or legal guardian, hereby grant permission for the minor named above to participate in the activity described in the Scope of Activity. I authorize the Organization and its agents to provide or arrange for routine first aid and emergency medical treatment as reasonably necessary. If emergency medical treatment is required, I authorize medical personnel to treat the minor and agree to be responsible for any costs incurred.

I understand and acknowledge the risks inherent in the activity. In consideration of the minor being permitted to participate, I, on behalf of myself, my heirs, assigns, personal representatives and next of kin, hereby release, waive, discharge and covenant not to sue the Organization, its officers, directors, employees, volunteers and agents from any and all liability, claims, demands, actions or causes of action arising out of or related to any loss, damage, injury or death that may be sustained by the minor, whether caused by negligence of the Organization or otherwise, to the fullest extent permitted by law.

Governing Law

If any provision of this Release is determined to be invalid or unenforceable, such provision shall be severed and the remainder of the Release shall remain in full force and effect.
