What the Passport Verification Form Is and when it's used

The Passport Verification Form documents an identity check of an individual using passport data and supporting evidence. It records key passport fields, confirmation steps (visual check, photocopy, or electronic verification), the verifier's identity, and any notarization or remote-online-notarization (RON) credentials required by the receiving organization.

Why a completed Passport Verification Form matters

A correctly completed form reduces identity-related delays, supports compliance for regulated transactions, and creates a clear audit trail for later review or dispute resolution.

Why a completed Passport Verification Form matters

Who commonly completes and relies on this form

Use by these groups supports regulatory compliance (e.g., KYC, HIPAA recordkeeping) and reduces downstream disputes over identity.

  • Real estate closing agents and landlords verifying tenant identity for lease and closing documents.
  • Healthcare intake staff confirming identity before releasing records or linking patient accounts.
  • Financial institutions and lenders performing identity verification for account opening and KYC purposes.

Stepwise process to complete the Passport Verification Form

Follow these sequential steps to collect and record passport verification clearly and defensibly.

  • 01
Collect ID: Ask for passport and a secondary ID if required.
  • 02
Compare data: Visually match name, DOB, and photo to the applicant.
  • 03
Record fields: Enter passport number, issuing country, and expiration date.
  • 04
Sign and notarize: Have verifier sign; notarize or use RON per policy.

How electronic completion and submission typically flow

Digital workflows streamline collection while preserving audit evidence; below are the common stages in an eSubmission process.

  • Upload document: Sender uploads PDF or DOCX form to signing platform.
  • Add fields: Place name, date, signature, and optional notary fields.
  • Authenticate signer: Use email, SMS code, or stronger ID-check methods.
  • Complete and archive: Signed copy and audit trail are stored securely.

Platform and technical considerations for eSubmission

Ensure the platform supports audit trails, secure storage (AES-256), and any required BAAs or 21 CFR Part 11 controls for your use case.

Verification timing:

Complete at the point of identity collection

Passport fee due:

Pay execution fee at application submission ($35)

RON record retention:

Audio‑video typically retained 5–10 years

Provide copy:

Deliver signed copy to subject promptly

Audit trail retention:

Keep digital audit evidence per policy

Common mistakes to avoid when preparing the form

  • Transcribing names or passport numbers incorrectly, causing verification mismatches and rejected submissions.
  • Accepting expired passports without supplemental ID, which fails many acceptance policies and regulatory checks.
  • Omitting notary or RON session identifiers when required, leaving verification evidence incomplete.
  • Relying on low-assurance authentication (email only) for high-risk transactions, increasing fraud exposure.

Risks and potential consequences of incorrect verification

Transaction delay: Lost or postponed processing
Regulatory breach: Noncompliance exposure
Financial loss: Fraud-related charges
Legal liability: Civil claims possible
Record rejection: Government may refuse filing
Data breach: PII compromise risk
