By signing below, Payer authorizes Payee and its agents to initiate debit entries to the payment account specified above for the amounts and frequency indicated. This authorization permits the Payee to charge the Payer's selected account for authorized amounts, applicable taxes, shipping and handling where provided, and any agreed fees. Payer represents and warrants that the information provided is true and that Payer is an authorized signer on the designated payment method.

Payer agrees that Payee may process charges electronically and may store card or bank account data for purposes of recurring transactions and authorization verification. Payer understands that this authorization will remain in effect until Payee receives written notice of revocation at least thirty (30) days prior to the next scheduled payment. Revocation does not affect the validity of previously authorized charges.

If any debit is returned unpaid, Payer will be responsible for returned item fees, collection costs, and interest where permitted by law. Disputed charges must be submitted in writing within sixty (60) days after the date of the transaction. Payer acknowledges that chargebacks for authorized transactions may be subject to additional fees and that Payee may suspend service or take collection action for nonpayment.

Payer further agrees to indemnify and hold Payee harmless from any loss, claim, damages, or expense arising from Payer's breach of representations or failure to provide accurate payment information. This authorization is governed by applicable federal and state laws governing electronic funds transfers.
