Authorization: Payer expressly authorizes Payee to initiate debit entries to the designated bank account or charge entries to the designated card for the amounts and on the dates indicated. This authorization remains in effect until Payee receives written notice of revocation as described below and a reasonable opportunity to act (a minimum of the notice period specified below).

Returned Items and Fees: Payer agrees to pay all fees associated with returned items, including but not limited to insufficient funds fees, and any reasonable fees charged by Payee for collection of unpaid amounts. If a debit is returned unpaid, Payee may retry the debit and may assess a returned item fee of .

Cancellation/Revocation: To revoke this authorization, Payer must provide written notice to Payee. The required notice period is days prior to the next scheduled debit. Revocation does not relieve Payer of liability for payments already authorized and processed prior to revocation taking effect.

Liability and Indemnity: Payer represents that the information provided is true and accurate and that Payer is authorized to provide the payment method details. Payer will indemnify and hold Payee harmless from all losses, claims, liabilities, and expenses arising from incorrect or fraudulent information supplied by Payer.

Electronic Records and Notices: Payer consents to receive billing statements, notices of changes to payment amounts or schedule, and other communications related to this authorization electronically or at the email address provided above.
