By signing this Payment Debit Order Form, the payer authorizes the Payee to debit the bank account specified on this form in accordance with the debit instruction. This authorization constitutes an instruction to the payer's financial institution to effect payment of the debits and to act on any amendment or cancellation notice received from the Payee as provided below.

The payer acknowledges that the Payee may submit variable debits where the amount type selected permits this, provided the payer is given reasonable prior notification of the amount and date. The payer agrees that the Payee may collect reasonable administration and dishonour fees in the event a debit is returned unpaid, and the payer will be liable to the Payee for all costs of collection, including legal fees on an attorney-and-client scale where permitted by law.

Cancellation or variation of this mandate must be made in writing and received by the Payee at least thirty (30) days prior to the next scheduled debit. The payer acknowledges that the Financial Institution may require the payer to instruct the bank directly and that the Financial Institution will not be liable for debts not honoured where the payer's account does not have sufficient funds or where account details provided are incorrect.

The payer indemnifies the Payee and the Financial Institution against any claim, loss or damage arising from incorrect information supplied by the payer, unauthorized amendments, or from any act or omission by the payer that causes a debit to be dishonoured.
