By signing below, the account holder authorizes the Biller named above to initiate debit entries to the bank account identified on this form and, if necessary, to initiate correcting credit entries. This authorization remains in effect until the Biller receives written notice of revocation at least 30 calendar days prior to the next scheduled debit. Revocation will not affect debits already initiated prior to the effective date of revocation.

The account holder represents and warrants that they are an authorized signer on the account and have authority to authorize debits from the account. The account holder agrees to promptly notify the Biller of any change in account information or termination of this authorization. The Biller is not responsible for bank processing delays beyond Biller's control.

Returned or dishonored items may be assessed a returned item fee as set by the Biller and may be re-presented for payment. The account holder agrees to reimburse the Biller for reasonable collection and legal costs incurred to collect any unpaid amounts resulting from returned items, except where prohibited by law.

The account holder authorizes the Biller to share or verify the account and debit information with financial institutions as necessary to effectuate payment or to resolve disputes. The Biller will obtain necessary approvals from its financial institution prior to submitting ACH debits and will comply with applicable rules governing electronic funds transfers.
