By submitting this Payment Disbursement Request, the Requestor certifies that: the goods or services represented by this request have been received or performed in accordance with the underlying agreement; the amounts requested are accurate and properly authorized; sufficient budget or appropriation exists to cover the payment; and the Requestor will cooperate with Accounts Payable in providing supporting documentation and with any subsequent audit.

The Requestor further authorizes the finance department to make payment in accordance with the payment method indicated. Requestor acknowledges that any misrepresentation, fraudulent documentation, or failure to obtain required approvals may result in reversal of payment, recovery actions, and disciplinary measures including termination and civil or criminal prosecution where applicable.

Accounts Payable reserves the right to withhold or suspend payment pending validation of supporting documentation, reconciliation of amounts, or resolution of disputes. Acceptance of payment by the payee constitutes a release of claims related to the paid invoice(s) to the extent of amounts paid.
