What the Payment EFT and Statement Does

Payment EFT and Statement is a combined authorization and remittance record used to initiate electronic funds transfers (EFT) and to provide a formal statement of payment details. The form typically includes payer and payee identification, bank routing and account numbers, payment amount, payment date, invoice or reference numbers, and authorization language permitting ACH or wire transfers. Organizations use it to collect banking instructions securely, document agreed transfers, and reconcile payments. When completed accurately, it supports automated settlement, audit trails, and record retention for financial, tax, and compliance purposes.

Why a Standardized Payment EFT and Statement Matters

Payment EFT and Statement standardizes transfer instructions, reduces payment errors, and creates an auditable record for reconciliation and compliance. Accurate statements support tax reporting, reduce duplicate payments, and make funds traceable under ESIGN and banking regulations.

Why a Standardized Payment EFT and Statement Matters

Who Completes and Who Receives This Form

Payment EFT and Statement is completed by finance, accounts payable, vendors, and authorized signers to authorize and document electronic payments.

  • Accounts payable teams — prepare EFT instructions, match invoices, and schedule transfers.
  • Vendors and suppliers — provide bank details and authorize deposit of received payments.
  • Treasury or finance officers — review authorizations, approve wires, and maintain audit records.

Use role-based approvals and maintain access controls to limit who can submit or change EFT instructions.

Track submission cutoffs for bank processing, verification lead times, and tax reporting deadlines to avoid penalties and payment delays.

EFT Submission Cutoff:

Bank cutoff times vary; same-day ACH often requires submission by late afternoon local.

Vendor Setup Lead Time:

Allow 3–5 business days for bank verification and test transactions before scheduled payments.

Tax Reporting Deadlines:

1099-NEC to recipients and IRS due Jan 31 annually (IRC §6721).

Periodic Reconciliation:

Reconcile statements monthly within 30 days of receipt to detect returns or discrepancies.

Revocation Notice Period:

Require written revocation and bank notification; allow processing time before next scheduled transfer.

Common Preparation and Processing Mistakes to Avoid

  • Entering incorrect routing or account numbers, which commonly causes returned transfers and bank fees and delays reconciliation by days to weeks.
  • Mismatched legal names between payee record and bank account, leading banks to reject verification and delay funds until corrected.
  • Failing to obtain a completed W-9 or tax identification, which can trigger backup withholding obligations and tax reporting complications.
  • Using weak signer authentication or no audit trail, which undermines evidentiary value and increases exposure to fraud claims.

Regulatory and Financial Risks of Errors or Omissions

1099 Penalties: Late filing $60–$330 per form
Intentional Disregard: $660+ per form, no cap
Backup Withholding: 24% withholding for missing/incorrect TIN
I-9 Violations: $281–$2,789 per violation
Returned Transfers: Bank fees and collections delays
