By enrolling in Payment EZ Pay and signing below, Customer authorizes Issuing Party to charge the selected payment method for the amounts specified on this invoice and for recurring installments as scheduled. Customer affirms that the account information provided is accurate and that Customer has the authority to approve transactions on the selected account.

Late Payment: Payments not received by the due date are subject to a late fee equal to the greater of $25 or 1.5% of the outstanding balance per billing cycle. Returned Items: A returned payment fee of $35 will be charged for any check or ACH return. Default: If Customer defaults on scheduled payments, Issuing Party may accelerate the remaining balance, suspend services, or refer the account for collection; Customer is responsible for collection costs, including reasonable attorney fees.

Cancellation and Changes: To cancel or change an authorization for recurring payments, Customer must provide written notice at least 10 business days prior to the next scheduled draft. Issuing Party will make reasonable efforts to accommodate requests; however, cancellation does not relieve Customer of amounts already due or processed.
