What a Payment Pre-Auth Debit Request Is

A Payment Pre-Auth Debit Request is a written authorization that permits a payee or merchant to initiate electronic debits from a payer's bank account for a specified amount, schedule, and purpose. It typically captures payer identity, bank routing and account numbers, debit amount and frequency, effective date, and explicit revocation instructions. In U.S. use the form for one-time or recurring ACH/withdrawal arrangements and ensure any electronic signature meets federal and state e-signature rules such as the ESIGN Act and applicable state UETA or ESRA provisions.

Why a Clear Pre-Authorization Matters

A well‑crafted Payment Pre-Auth Debit Request reduces disputes, supports compliance with electronic signature laws, and documents consent for debit origination.

Why a Clear Pre-Authorization Matters

Who Typically Completes This Form

Businesses and service providers who collect ACH or debit payments use this form to document payer consent and bank instruction.

  • Real Estate: landlords and property managers collect rents and recurring fees with documented authorization and payment schedules.
  • Financial Services: loan servicers and billing departments collect installments, fees, or subscriptions with verified bank instructions.
  • Healthcare and Legal: providers and firms set recurring patient or client payment plans while capturing required consent language.

Use the form whenever an organization needs clear, auditable authorization for recurring or variable debits from a payer’s checking or savings account.

Essential Elements Every Request Should Include

A professional Payment Pre-Auth Debit Request is precise about identity, banking, payment terms, consumer rights, and how to cancel or dispute debits.

Payer Identity

Full legal name, contact phone, and mailing address to link authorization to account holder identity and for dispute notifications.

Bank Details

Clear routing and account numbers with account type and an optional voided check attachment to confirm bank instructions and prevent processing errors.

Payment Terms

Exact amount, variable maximum if allowed, frequency, start date, and any end date or specific invoice references for reconciliation.

Authorization Wording

Explicit consent language naming the payee, describing debit method, and stating that the payer authorizes electronic debits from the specified account.

Revocation & Notices

Instructions for how the payer may revoke authorization, required notice period, and where to send revocation communications for processing.

Signature Evidence

Signed and dated signature block or compliant eSignature, plus retention of the audit trail showing intent, attribution, and consent per ESIGN/UETA.

Common Preparation Errors to Avoid

  • Missing explicit authorization language or an unclear payee name leads to higher dispute rates and potential return of funds when challenged by the payer.
  • Transposed or incomplete routing and account numbers cause returned debits, bank penalties, and additional fees for reprocessing or manual corrections.
  • Failing to document the signer’s identity verification method increases fraud risk and weakens legal enforceability in a dispute or investigation.
  • Not specifying revocation procedures or maximum variable amounts creates ambiguity that can lead to consumer complaints or regulatory scrutiny.

Potential Consequences of an Incorrect Authorization

Unauthorized Debits: May trigger consumer refunds
Bank Chargebacks: Returned-debit fees and reversals
Regulatory Action: Possible enforcement or fines
Civil Liability: Damages and legal costs
Operational Delay: Manual investigations required
Reputational Harm: Loss of customer trust

Processing Timelines and What to Expect

Set expectations for submission lead times, bank processing, and how revocations or disputes affect scheduled debits.

Submission Lead Time:

Submit authorizations well before the first scheduled debit to allow verification and bank processing.

Bank Posting Window:

Banks typically process debits in standard ACH cycles; originator timing determines posting date.

Effective Date:

The start date on the form is when debits may begin, subject to bank cutoffs and verification.

Revocation Notice:

Revocations take effect when processed; allow reasonable time for originator and bank to implement.

Record Retention:

Keep signed authorizations and audit trails for required retention periods for dispute resolution.
