I, the undersigned employee, authorize Employer named above to deduct from my wages the amounts indicated in this Payroll Deduction Authorization. Deductions shall be made in accordance with the frequency selected and in conformity with Employer payroll procedures and applicable law. This authorization applies to all future wages until the obligation is satisfied or I provide written revocation as set forth below.

I understand that certain deductions (including pre-tax benefits, retirement contributions, and court-ordered garnishments) are governed by plan documents or legal process and may be processed in accordance with those requirements. This authorization does not supersede any court order, statutory requirement, or prior written agreement that directs payroll withholding.

I certify that the information provided is true and accurate. I agree to notify Employer in writing of any change in my status that affects this authorization. I understand that Employer requires reasonable time to implement or terminate deductions, and I authorize deductions to be made during that processing period. I release Employer from liability for properly implementing deductions made in good faith in accordance with this authorization, except for damages resulting from Employer's gross negligence or willful misconduct.

For loan repayments or payroll advances, I authorize Employer to adjust final deduction amounts to reflect the outstanding balance and any applicable interest or fees authorized by a separate loan agreement. If an overpayment or erroneous payment occurs, I authorize Employer to recover such amounts by payroll deduction or other lawful means.

Revocation: I may revoke this authorization by providing written notice to Employer's payroll or human resources department. Revocation will be effective only after Employer has had a reasonable opportunity to process the request and will not affect amounts properly withheld prior to revocation.
