What the Pension Transfer Agreement Form Is and when it applies

The Pension Transfer Agreement Form formalizes the transfer of pension benefits, rights, and account ownership between a transferring participant or plan sponsor and the receiving party. It names all parties, identifies the specific plan and account numbers, states the effective transfer date, and records the transfer method (rollover, lump-sum, assignment) and any consideration. The form also records tax treatment elections, beneficiary updates, and required authorizations so trustees can validate the transfer and complete necessary IRS reporting.

Why documenting a pension transfer matters
