What the Performance Evaluation Addendum Is

A Performance Evaluation Addendum is a written attachment to an existing employment agreement or personnel file that documents evaluation results, changes to performance expectations, corrective actions, or revised goals. It records the outcome of a formal review, clarifies next steps for the employee and manager, and becomes part of the employment record when signed by the required parties. The addendum may be used for development planning, disciplinary steps, or as supporting documentation in HR or legal proceedings.

Why this Addendum Matters for Employers and Employees

A clear addendum ensures both parties share the same understanding about performance findings, expectations, and remedies. It reduces disputes by documenting agreed actions and timelines, supports consistent HR processes, and creates an auditable record for compliance and future decisions.

Why this Addendum Matters for Employers and Employees

Who Typically Prepares and Signs This Addendum

Human resources, direct managers, and legal or compliance teams commonly prepare or approve a Performance Evaluation Addendum before presenting it to the employee.

  • Managers and supervisors who conduct the performance review and outline expectations and corrective actions.
  • HR professionals who verify policy alignment, retention, and file placement for personnel records.
  • Employees who acknowledge receipt, agree to development plans, or record disputes and responses.

The document is intended for internal use but may be produced in employment disputes; accurate completion and secure storage matter.

Typical Signatories and Their Roles

HR Manager

An HR Manager reviews the addendum for policy compliance, ensures proper approvals are attached, and files the signed document in the official personnel record. They may coordinate witness or notary requirements if the organization’s policy or state law requires it.

Employee

The employee receives the addendum, reviews the findings and required actions, and signs to acknowledge receipt. Signing does not always equal agreement; the employee may attach comments or a written response for the record.

Core Elements to Include in a Professional Addendum

A well-drafted Performance Evaluation Addendum contains specific sections that make the record clear, actionable, and defensible.

Scope

Define whether the addendum modifies an existing agreement or documents a separate review event, and state which employment terms it affects.

Evaluation Period

Specify the exact dates covered by the review so the findings are time-bound and linked to documented performance metrics.

Performance Metrics

List concrete, measurable indicators used in the evaluation, avoiding vague or subjective language that is hard to verify later.

Required Actions

Describe corrective measures, training, or milestones with deadlines and responsible parties for each follow-up step.

Consequences

Explain possible outcomes for failure to meet corrective measures, including disciplinary options or changes in employment status.

Signatures

Provide dated signature blocks for the employee, manager, and any HR or witness signers required by policy or law.
