What a Personal Acquaintance Form Is

A Personal Acquaintance Form documents an individual's direct knowledge of another person's identity, character, or background for administrative, screening, or verification purposes. It typically records the declarant's name, relationship to the subject, factual observations, relevant dates, and a signature block; it can be used as a reference for employment, tenancy, licensing, or compliance checks.

Why this form matters and what it achieves

A clear Personal Acquaintance Form provides contemporaneous evidence of personal knowledge that reduces ambiguity in decisions, supports KYC and background checks, and documents factual observations that can be reproduced or authenticated when questions arise.

Why this form matters and what it achieves

Who typically completes or requests this form

Organizations and individuals use this form when a third-party attestation of identity or character is required for screening, onboarding, or recordkeeping.

  • Landlords and property managers screening prospective tenants for references and local ties.
  • Employers or hiring managers verifying character or community references during background checks.
  • Legal or licensing bodies needing a written acquaintance statement as part of an application or investigation.

Use the form only where a personal attestant has direct, first-hand knowledge; the form is not a substitute for official identity documents or formal background checks.

Essential parts of a professional Personal Acquaintance Form

A concise, well-structured form groups factual elements so statements are traceable, minimizes ambiguity, and supports authentication. The following components appear in most professional templates.

Declarant ID

Full legal name, contact details, and relationship to the subject documented clearly for follow-up and attribution purposes.

Subject ID

Full legal name and identifying details of the person being described; avoid nicknames and include date of birth or other identifiers when appropriate.

Acquaintance Basis

A factual description of how and for how long the declarant has known the subject, including dates and context of interactions.

Observations

Specific, date-referenced statements about character, conduct, or facts—avoid opinions framed as facts and be as concrete as possible.

Signature

Declarant signature and printed name with date; include signer role and contact method to permit re-verification.

Authentication

Notary acknowledgement or witness block when required by requester or jurisdiction to strengthen evidentiary weight.

Required information at a glance

Declarant name: Full legal name
Declarant contact: Phone and email
Subject name: Full legal name
Acquaintance dates: From and to dates
Signed date: MM/DD/YYYY
Witness/Notary: If required

Step-by-step: filling and submitting the form

Follow this sequence to complete the form accurately, authenticate the statement, and submit it to the requesting party.

  • 01
Gather ID: Collect government-issued ID details to ensure the declarant's name matches official records.
  • 02
Write facts: Complete the acquaintance basis and observations with clear dates and concrete examples.
  • 03
Sign and date: Have the declarant sign and date in MM/DD/YYYY format; include contact information.
  • 04
Authenticate: If requested, notarize or obtain witness signatures per the requester’s or jurisdiction’s rules.

Configuring an online workflow for the form

When sending the form electronically, set the template fields, signer order, and authentication to match the requester's requirements.

Field Configuration
Template Lock required fields and enable conditional fields for optional sections
Signing order Set declarant first, then witness/notary if applicable
Authentication Email + SMS code or KBA for higher assurance
Retention Enable automatic storage and audit trail for compliance

Where to send or file the completed form

Route the signed form to the requester and keep a copy for your records; eSubmission options depend on the recipient’s requirements.

  • Submit to requester: Email or secure upload to the organization that requested the form
  • Store copy: Retain a signed copy in your records for the applicable retention period
  • Notary office: If notarization required, submit original to notary for acknowledgement
  • Third-party verifier: Provide authenticated copy to background check vendor when requested

Technical and platform considerations for eSubmission

Choose a platform that supports required authentication, audit trails, and file formats to preserve evidentiary quality.

  • Integrations: Salesforce | NetSuite | Google Workspace
  • File formats: PDF | DOCX | HTML
  • Authentication: Email link, SMS code, or KBA

Typical timelines and processing expectations

Timelines depend on the requester; plan for verification and authentication steps when scheduling decisions that rely on the form.

Provide on request:

Submit the form when requested by landlord, employer, or licensing authority; no universal filing deadline

Allow verification:

Expect 3–10 business days for follow-up verification by requesters

Notary scheduling:

If notarization required, factor in scheduling a notary and possible travel fees

Retention copy:

Keep a signed copy immediately; scanning and saving reduces loss risk

Respond to queries:

Respond promptly to verification requests to avoid delays in the underlying application

Common mistakes to avoid when preparing the form

  • Using informal names or nicknames instead of legal names, which can cause identity mismatches and delays in verification.
  • Submitting unsigned or undated forms; an unsigned declaration is often treated as invalid by requesters or courts.
  • Providing vague or opinion-based statements rather than concrete, date-referenced facts that can be corroborated.
  • Failing to confirm whether notarization or witness signatures are required by the requester or local law.

Potential consequences of errors or misstatements

Invalid signature: Form may be rejected
Mismatched identity: Triggers additional verification
Unsanctioned disclosure: Privacy or HIPAA risk
Missing notarization: Reduced evidentiary weight
Late response: Delays the decision process
False statement: Potential civil or criminal liability
