Cancellation, Rescheduling and Refunds

Cancellations must be made in writing. Client agrees to provide at least days' notice for scheduled sessions. Sessions cancelled with less than the required notice are forfeited unless Coach elects to provide a credit. Deposits are non-refundable except as expressly stated below.

Rescheduling requests will be accommodated subject to Coach availability. If Coach cancels, Client will be offered a prompt replacement session or refund for the affected session(s).

Confidentiality and Privacy

Coach will maintain confidentiality of Client information shared during coaching sessions except where disclosure is required by law or reasonably necessary to prevent imminent harm. Coach may collect and store contact and billing information to administer services. Client consents to such collection and to Coach's retention of necessary records for professional and legal compliance.

Intellectual Property and Materials

All course materials, written content and intellectual property provided by Coach remain the exclusive property of Coach. Client is granted a limited, non-transferable license to use materials for personal, non-commercial purposes only. Client will not reproduce, publish or distribute materials without Coach's prior written consent.

Limitation of Liability and Disclaimer

Client acknowledges that coaching is a professional relationship focused on personal development. Coach does not diagnose or treat mental health conditions and is not a substitute for medical or mental health professionals. Coach makes no guarantees regarding results. To the maximum extent permitted by law, Coach's liability for any claim arising from services is limited to the amount of fees actually paid by Client under this Order.

Termination

Either party may terminate this Agreement by providing written notice. Termination by Client does not relieve Client of the obligation to pay for services already rendered. Coach may terminate immediately for nonpayment or for conduct that compromises safety or professional boundaries. Upon termination, outstanding balances become immediately due.

Indemnification

Client agrees to indemnify and hold harmless Coach from any claims, losses or liabilities arising out of Client's breach of this Order, negligence, willful misconduct, or misuse of coaching materials.

Acknowledgment and Consent

By signing below, Client and Coach agree that the terms set forth in this Personal Coaching Order Form constitute the entire agreement for the services described and supersede prior discussions or representations. Client confirms that they are entering into coaching voluntarily and have provided true and complete information as required for delivery of services.

Client acknowledges receipt of these terms and agrees to pay the fees as specified.
