Activation Terms and Conditions

By activating this Personal Trial Pass, the pass holder receives the limited, non-transferable trial described above for the trial period only. The trial pass grants only the specific access described in the Access Scope and is subject to facility rules, instructor policies, and code of conduct. The pass holder acknowledges that the provider may revoke access for violation of rules or for health and safety concerns.

Automatic Conversion: Unless the pass holder cancels prior to the Trial Expiration Date, this trial will automatically convert to a paid membership or paid access plan under the provider's standard terms and fee schedule. The pass holder hereby authorizes collection of fees upon conversion using the payment method on file or otherwise provided in the Payment Authorization section below.

No-Transfer; Single Use: The trial pass is personal to the pass holder and may not be sold, transferred, shared, or used by any person other than the pass holder named in this document. Violation permits immediate termination of the pass without refund.

Payment Authorization

Payment Method on File (last four digits or description):

Authorization to Charge: I authorize the provider to charge my payment method for any fees that become due upon conversion of the trial to a paid plan and for any late cancellation or no-show fees as described in provider policy. This authorization survives termination of the trial and applies to recurring charges if the trial converts to a recurring membership.

I consent to the Payment Authorization and automatic charges as described above.

The pass holder represents and warrants that the information provided is true and complete, that the pass holder is at least 18 years of age (or has legal guardian consent if younger), and that the pass holder has not previously received an identical trial promotion unless expressly permitted. The pass holder consents to collection and use of contact information for account administration and notifications related to the trial and possible membership conversion.

Communications: By providing contact information the pass holder agrees to receive email or text communications regarding account status, trial reminders, billing notices, and promotional communications. Standard message and data rates may apply.

Release and Limitation of Liability

To the fullest extent permitted by law, the pass holder releases and holds harmless the provider, its employees, agents, and affiliates from any liability for personal injury, property loss, or other damages arising from participation in activities covered by this trial pass, except to the extent caused by the provider's gross negligence or willful misconduct. The pass holder acknowledges the inherent risks of physical activity and assumes responsibility for personal safety.

Cancellation, Refunds and Disputes

Cancellation must be made in writing to provider contact channels before the Trial Expiration Date. If cancellation is timely, no charges will be applied. If the pass converts and a charge posts, refund eligibility is governed by the provider's refund policy as applied to converted memberships; refunds are not guaranteed. Disputes regarding billing must be raised within 60 days of the charge.

By signing below the pass holder acknowledges receipt of this Trial Pass Activation form, confirms understanding of the trial period and automatic conversion terms, and certifies that all information provided is accurate. The pass holder understands that activation is effective as of the Trial Start Date entered above and that termination of the trial prior to conversion must be timely to avoid charges.

What the Personal Trial Pass Activation Is

A Personal Trial Pass Activation is the process and record that grants an individual temporary access to a service, product trial, or platform feature under predefined terms. The activation record documents the account holder, activation date, trial duration, access scope, and any trial-specific limitations such as feature caps or usage quotas. In many U.S. contexts the activation may include consumer-facing disclosures required by the ESIGN Act when an electronic record replaces a paper notice; the activation record also serves as an auditable event for compliance, billing, and support workflows.

Why a Clear Activation Record Matters

A well-formed Personal Trial Pass Activation reduces billing disputes, supports customer service, and preserves evidence of consent for electronic transactions under ESIGN and applicable state rules.

Why a Clear Activation Record Matters

Who Typically Completes a Trial Pass Activation

The Personal Trial Pass Activation is completed by the end user or an authorized administrator depending on whether the trial is self-service or provisioned by staff.

  • End users enrolling for a single-user trial with personal credentials and consent.
  • IT or product administrators provisioning trial accounts for internal evaluation or pilot groups.
  • Sales or support staff creating activation records when onboarding prospective customers.

Accurate completion ensures clear start/end dates, consent documentation, and reduces friction at conversion or termination.
