What the Personal Tune Up Program Is

The Personal Tune Up Program is a standardized intake and agreement package designed to document a short-term review, assessment, and action plan for an individual's professional or personal goals. It captures identifying data, current status, prioritized objectives, recommended steps, scheduling preferences, and consent for follow-up communications. The package is suitable for use by coaches, consultants, health professionals, and HR teams who require a recorded baseline and documented commitments. It supports electronic completion, optional notarization, and structured retention for compliance and audit purposes.

Why the Program Matters for Accurate Records

Use the Personal Tune Up Program to create a concise, auditable record of an assessment and agreed next steps. It clarifies responsibilities, documents consent, reduces follow-up ambiguity, and provides a single source of truth for progress reviews and regulatory or organizational recordkeeping.

Why the Program Matters for Accurate Records

Typical Users and Contexts

Common users include coaches, consultants, HR teams, and clinical or wellness providers who need documented assessments and action plans.

  • Independent coaches and consultants managing short-term client engagements and measurable goals.
  • HR or talent teams using structured tune-up sessions for onboarding, performance plans, and check-ins.
  • Healthcare or wellness practitioners documenting baseline assessments and patient or participant follow-up consent.

Completed programs support scheduling, follow-up tracking, and retention under organizational or regulatory policies to maintain auditability.

Representative Signers and Administrators

Brian Fitzgibbons, COO

As COO of a small services firm, he uses the Personal Tune Up Program to centralize client intake, document agreed milestones, and reduce administrative follow-up. The standardized form helps his teams track completion and maintain an auditable history for billing and compliance.

Tim Martin, Founder

A real estate operator uses the program to capture tenant or contractor commitments, schedule touchpoints, and attach exhibits. Digital completion and audit trails reduce in-person meetings while producing a clear record for dispute resolution or project handoff.

Essential Elements of a Professional Program

Core components of a professional Personal Tune Up Program establish scope, responsibilities, tracking fields, and integration with storage and audit processes.

Intake Form

A standardized intake captures legal name, contact details, health or work status, and consent language. Structured fields reduce ambiguity and enable automated routing and reporting across teams.

Action Plan

A clear, dated action plan assigns tasks, owners, and deadlines. Including measurable outcomes supports objective review and anchors subsequent follow-up sessions and performance measurement.

Consent & Privacy

Explicit consent section documents data-sharing preferences and disclosures required by consumer-facing rules. Include a record of consent acceptance and withdrawal procedure for ESIGN compliance.

Scheduling

Integrated scheduling fields record follow-up dates, reminder preferences, and rescheduling rules. This reduces manual coordination and preserves timing evidence for audits or disputes.

Attachments

Attach supporting documents, assessments, or exhibits as appendices. Properly labeled exhibits maintain context and should be stored with the signed record in PDF/A or equivalent archival format.

Audit Trail

A detailed audit trail records signer identity, timestamps, IP addresses, and field changes. This metadata supports enforceability and demonstrates chain-of-events in case of challenge.

Four Steps to Complete the Program

Complete the Personal Tune Up Program in four steps to ensure accurate intake, documented consent, and scheduled follow-up.

  • 01
Prepare Details: Gather ID, contact, and baseline information before starting.
  • 02
Set Objectives: List prioritized goals and measurable outcomes with dates.
  • 03
Review & Consent: Review terms, data use, and sign consent fields.
  • 04
Confirm Schedule: Select follow-up dates and responsible parties for reviews.

How to Configure the Online Workflow

Configure the online form to enforce required fields, set conditional logic, and route completed records to designated storage locations and approvers.

Field Configuration
Required Fields Full name; email; date; consent checkbox
Conditional Logic Show follow-up fields when consent is given
Signer Authentication Email link or SMS code; optional KBA
Storage Destination Secure cloud folder; PDF/A archival recommended

Typical Digital Signing Flow

The digital workflow routes the program to stakeholders, captures signatures, and stores a complete audit trail for future reference.

  • Upload Document: Start from a PDF or DOCX template and upload into the workflow.
  • Add Fields: Place signature, date, and conditional fields as required.
  • Invite Signers: Enter email addresses or create a secure signing link.
  • Finalize: Complete signing, capture audit trail, and download final document.

Delivery Channels and Platform Needs

The Personal Tune Up Program can be shared by email, secure link, integration, or in-person kiosk depending on platform capabilities.

  • Integrations: Salesforce, NetSuite, Google Workspace supported
  • Formats: PDF, DOCX, HTML supported
  • Authentication: Email, SMS, SSO, and optional KBA

Common Preparation Mistakes to Avoid

  • Collecting incomplete contact or identification data, such as using nicknames or partial addresses, which causes processing delays and verification failures.
  • Failing to obtain explicit consent for electronic records when required for consumer-facing services, which can invalidate e-consent under ESIGN.
  • Placing initials where full signatures are required, or omitting dates, resulting in ambiguous effective dates and enforcement challenges.
  • Attempting to notarize an e-signed copy without following state RON or journal retention rules, creating a possible authentication gap.

Consequences of Incorrect or Incomplete Records

Incorrect TIN: Triggers 24% backup withholding
Late reporting: $60–$330 per form penalties
Intentional disregard: $660+ per form, no cap
I-9 violations: $281–$2,789 per violation
Privacy breach: HIPAA fines and remediation
Invalid signatures: Possible unenforceability or dispute
