I grant permission to the Released Parties to take and use photographs, video, and audio recordings of me and my likeness for promotional, educational, or archival purposes, in any medium, without further consent or compensation. I understand that such media may be edited and used at the Released Parties' discretion.

If the participant is under 18 years of age, a parent or legal guardian must sign on the participant's behalf. By signing, the parent or guardian represents that they have the authority to act on behalf of the minor and agree to the terms of this Waiver and Release.
