Start Date:    End Date (if any):

Either party may terminate this Agreement for material breach by the other party if the breach is not cured within the Notice Period after written notice. Performance Standards; Access and Conditions

Contractor shall perform services in a professional manner consistent with industry standards. Client shall provide reasonable access to areas to be excluded and shall ensure utilities and conditions necessary for performance. Contractor shall not be responsible for rodent entry resulting from conditions beyond reasonable exclusion (for example, continued accumulation of attractants, unauthorized structural alterations, or failure to maintain recommended repairs).

Limited Warranty and Liability

Contractor warrants that exclusion work shall be free from defects in materials and workmanship for a period of ninety (90) days from completion, subject to Client compliance with recommended ongoing maintenance. This warranty does not cover damage or reinfestation due to Client's failure to maintain premises or new entry points created by third parties. Contractor's total liability for any claim arising out of this Agreement shall not exceed the amount paid by Client for the applicable services, and Contractor shall not be liable for incidental, consequential or punitive damages.

Confidentiality

Each party agrees to maintain in confidence any non-public business or technical information disclosed by the other party in connection with this Agreement and identified as confidential at the time of disclosure ("Confidential Information"). Confidential Information shall not include information that is or becomes publicly known through no wrongful act of the receiving party or is independently developed without use of the disclosing party's information. The confidentiality obligations survive termination of this Agreement for a period of two (2) years.

No waiver, modification or amendment of any provision of this Agreement shall be effective unless in writing and signed by both parties.

Acknowledgment

Client acknowledges receipt of this Agreement and the Scope of Work and consents to Contractor's performance under the terms set forth. Client further acknowledges that exclusions are intended to reduce entry points but do not guarantee permanent elimination of all rodents where structural or environmental conditions persist.

What the Pest Control Rodent Exclusion Form Is

The Pest Control Rodent Exclusion is a written scope-of-work and release used between a pest control provider and a property owner to document exclusion work for rodents. It defines entry-point repairs, exclusion materials and methods, responsibilities for follow-up, and any service limitations or warranty terms. The document clarifies who supplies materials, who performs structural repairs versus cosmetic fixes, inspection and verification steps, and the timeline for completion. Properly completed, it creates a record of agreed services and expectations for both parties.

Why a Rodent Exclusion Form Matters

A clear exclusion agreement reduces disputes by recording the agreed scope, materials, access, and warranty terms in writing. It helps protect property owners and contractors by allocating responsibility for repairs and ongoing monitoring.

Why a Rodent Exclusion Form Matters

Who Typically Uses This Rodent Exclusion

Use the form to ensure consistent expectations and to support billing, insurance, or warranty claims later.

  • Property owners and landlords who need written confirmation of exclusion scope and warranty terms.
  • Licensed pest control contractors who document the work performed, materials used, and limitations.
  • Property managers and HOAs who require standardized documentation across multiple units or properties.

Core Components of a Professional Rodent Exclusion

A complete exclusion document organizes the work, materials, responsibilities, timing, and legal protections into separate, clear sections for easy reference.

Scope of Work

Precise list of exclusion tasks and inspected areas; identifies interior and exterior entry points and structural repairs required, so there is no ambiguity about what is included.

Materials & Methods

Specifies approved sealants, mesh, vents, and exclusion techniques to be used; identifies materials that are excluded from the contract or require prior owner approval.

Access & Cooperation

Describes owner responsibilities for access, moving furniture, or authorizing third-party trades (carpentry, masonry) needed to complete structural exclusion work.

Inspection & Verification

Defines pre-work inspection, post-exclusion verification, and any follow-up visits including acceptable evidence of closure (photos, signed checklist, or inspection report).

Warranty & Limitations

States warranty length, what is covered (e.g., re-entry through previously sealed points), and exclusions such as damage from unrelated structural defects.

Costs & Payment

Breaks out labor, materials, subcontractor charges, and payment schedule, and notes responsibility for permits or third-party repairs if applicable.

Step-by-Step: Completing the Rodent Exclusion

Follow these steps to prepare, review, and execute a complete exclusion agreement.

  • 01
Inspect: Survey property and document entry points.
  • 02
Define Scope: List all exclusion tasks and materials.
  • 03
Agree Terms: Confirm cost, schedule, and warranty.
  • 04
Sign & Record: Obtain signatures and keep copies.

Setting Up an Online Exclusion Workflow

Configure your digital workflow to collect signatures, photos, and verification evidence in a single record.

Field Configuration
Signer Authentication Email plus SMS code for higher assurance
Photo Attachments Allow required image uploads with minimum resolution
Conditional Fields Show repair fields only when structural work is needed
Audit Log Enable automatic timestamps and IP capture

Digital Signing and File Format Considerations

Ensure your chosen solution supports exporting signed records and audit certificates that meet ESIGN and UETA requirements.

  • Document Formats: PDF and DOCX are industry-standard and widely accepted.
  • Integrations: Connects to systems like Salesforce or NetSuite for record-keeping.
  • Storage: Encrypted at rest and in transit.

Where Completed Forms Should Be Sent or Stored

Route signed exclusion agreements to the right recipients and systems to preserve evidence and support follow-up.

  • Owner Copy: Provide a signed copy to the property owner.
  • Contractor Records: Retain in contractor file for warranty and insurance.
  • Property Management: Send to manager for multi-tenant properties.
  • Cloud Archive: Store encrypted with audit trail for retention.

Typical Timelines and Key Deadlines

Common timeframes and due dates associated with exclusion work and contract obligations.

Service Scheduling:

Start within 7–14 days after agreement execution.

Completion Window:

Most exclusions complete within 1–3 business days depending on scope.

Follow-up Inspection:

Typically scheduled 7–30 days after exclusion.

Warranty Period:

Commonly 30–90 days for re-entry coverage unless stated otherwise.

Payment Due:

Per agreed schedule — often on completion or net 30.

Common Preparation and Documentation Mistakes

  • Vague task descriptions that omit specific entry-point locations and materials, causing scope disputes.
  • Missing contractor license or insurance details, which complicates claims and local compliance checks.
  • Failing to document owner responsibilities like moving items or authorizing subcontractors, delaying completion.
  • Not capturing before-and-after photos or an inspection checklist, weakening warranty enforcement or insurance evidence.

Principal Risks of an Incomplete or Incorrect Agreement

Regulatory Noncompliance: Local licensing fines
Insurance Denial: Claim disputes due to incomplete records
Warranty Disputes: Ambiguous coverage terms
Liability Exposure: Unaddressed structural hazards
Contractor Payment Risk: Unclear payment schedule
Property Damage Claims: Unspecified repair responsibilities

Practical Tips for Accurate and Efficient Completion

Apply consistent documentation and verification practices to reduce disputes and speed resolution.

Document Visual Evidence
Take dated before-and-after photos of all sealed entry points and attach them to the signed agreement to support warranty claims and insurance inquiries.
Use Specific Materials
Name approved sealants, meshes, and fasteners rather than generic terms to avoid disagreements about acceptable repairs and longevity.
Record Contractor Details
Include license numbers, insurance carrier and policy numbers, and primary contact information for rapid verification by owners or authorities.
Maintain an Audit Trail
Preserve timestamps, signer authentication logs, and versioned documents to meet ESIGN and UETA evidentiary expectations in disputes.

Example Use Cases

Real-world scenarios illustrate how a clear exclusion agreement reduces disputes and clarifies responsibilities.
