SCOPE OF WORK

The Adopter agrees to provide the following ongoing care, and the Shelter documents the animal's known needs. Describe responsibilities, routines, training expectations, and any special medical or behavioral care below.

PAYMENT TERMS

Adoption Fee Amount: $. The adoption fee covers administrative processing, basic vaccinations, and microchip registration where applicable.

Deposit Amount (if applicable): $ payable at time of application. Deposits are non-refundable except where Shelter, in its sole discretion, cancels the adoption prior to transfer.

Late Fee: If any scheduled payment is more than days late, a late fee of $ or of the outstanding balance (whichever is greater) will apply. Adopter agrees to reimburse Shelter for reasonable collection costs and legal fees incurred to enforce payment obligations.

TERM AND TERMINATION

Trial Period End Date (if any): . During the trial period the Adopter may return the animal to the Shelter with prior notice.

Notice Period for Termination by Either Party: days written notice. If the Adopter is unable or unwilling to care for the animal at any time, the Adopter must notify the Shelter and make arrangements to return the animal. The Shelter reserves the right to reclaim the animal if it reasonably determines the animal is not being properly cared for or if the Adopter materially breaches this Agreement.

CONFIDENTIALITY

The parties acknowledge that certain personal information (including adopter contact information and veterinary records) is confidential. Shelter will maintain adopter contact and animal medical records in accordance with its record retention policies and applicable law. Adopter consents to Shelter contacting the listed veterinarian(s) and references to verify care and vaccination history. Adopter's personal information shall not be disclosed except as required by law or as necessary to protect the health and welfare of the animal.

GOVERNING LAW; ENTIRE AGREEMENT

This Agreement shall be governed by and construed in accordance with the laws of the State of without regard to conflict of law principles. Venue for any dispute arising under this Agreement shall be in the courts located within that State.

This Agreement constitutes the entire agreement between the parties with respect to the subject matter herein and supersedes all prior or contemporaneous understandings and agreements, whether written or oral. Any amendment must be in writing and signed by both parties.
