PET CARE AGREEMENT

Recitals

WHEREAS, Client is the lawful owner of the animal(s) described in this Agreement and desires to retain Provider to perform pet care services under the terms set forth below; and

WHEREAS, Provider represents that Provider possesses the experience, staff, and facilities to provide such pet care services and agrees to perform services in a competent and professional manner consistent with industry standards;

NOW, THEREFORE, in consideration of the mutual covenants contained in this Agreement, the parties agree as follows.

Scope of Work

Provider shall provide the following pet care services for the pet identified above. The parties may specify frequency, duration, and special instructions in the fields below.

Payment Terms

Client agrees to pay Provider the fees described below. All fees are due according to the schedule set forth. Provider may suspend services if payment is delinquent in accordance with this Agreement.

Late Payment Fee: If any payment is more than days late, Client will pay a late fee of (flat amount or percentage per billing cycle).

Term and Termination

This Agreement commences on and continues until unless earlier terminated pursuant to this Section.

Either party may terminate this Agreement without cause upon delivering written notice to the other party at least days prior to the intended termination date. Provider may terminate immediately for non-payment or for material breach by Client. Termination does not relieve Client of payment obligations for services already performed.

Medical Authorization & Emergency Care

Client authorizes Provider to obtain veterinary care if, in Provider's reasonable judgment, emergency treatment is necessary for the pet. Provider will make reasonable attempts to contact Client and emergency contacts prior to non-urgent veterinary treatment.

Client authorizes emergency medical treatment up to the amount of $ without prior approval. Expenditures beyond this amount require prior approval except where delay would endanger the pet's life or well-being.

Liability, Indemnity, and Risk

Provider will exercise reasonable care for the pet while under Provider's custody. Client acknowledges risks inherent to animal handling and agrees that Provider shall not be liable for loss, injury, death, illness, or damage caused by the pet or to the pet arising from causes beyond Provider's control, including but not limited to pre-existing conditions, contagious diseases, or acts of third parties, except to the extent caused by Provider's gross negligence or willful misconduct.

Client shall indemnify and hold harmless Provider, its employees and agents from and against any claims, liabilities, losses, damages, costs and expenses (including reasonable attorneys' fees) arising out of Client's breach of this Agreement or Client's failure to disclose material information regarding the pet's health or behavior.

Confidentiality

Provider shall maintain the confidentiality of Client's contact information, pet medical records, and any proprietary instructions provided by Client, and shall not disclose such information except as required to provide services hereunder, as required by law, or with Client's prior written consent.

Governing Law and Dispute Resolution

This Agreement shall be governed by and construed in accordance with the laws of the state selected by the parties. Any dispute arising out of or related to this Agreement shall be resolved by binding arbitration if the parties so agree in writing, or otherwise in the courts of competent jurisdiction in the governing state.

Entire Agreement; Amendment

This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior discussions, representations, or agreements. No amendment or modification shall be effective unless in writing and signed by both parties.

If any provision of this Agreement is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
