What the Pet Care Canine Stars document is

The Pet Care Canine Stars is a standardized service agreement and intake form used by pet care providers to capture client instructions, pet health details, emergency contacts, service dates, and payment terms. It functions as a contract between the owner and the provider, documents liability allocations, and records consent for routine and emergency care. The form can be used in boarding, daycare, grooming, and in-home pet-sitting contexts and is designed to be completed in person or electronically with an audit trail for recordkeeping and compliance.

Why a clear Pet Care Canine Stars form matters

A complete Pet Care Canine Stars agreement reduces misunderstandings about services, clarifies emergency procedures, ensures handlers have medical and behavior details, and establishes payment and cancellation terms in writing to limit disputes.

Why a clear Pet Care Canine Stars form matters

Who typically completes or signs this form

The Pet Care Canine Stars form is completed by the pet owner and the receiving service provider before services begin.

  • Independent dog walkers and pet sitters capturing client preferences, keys, and emergency contacts.
  • Boarding and daycare facilities recording vaccination status, behavior notes, and authorized care.
  • Veterinary clinics and mobile care teams documenting consent for non-surgical emergency treatment.

Signed copies are retained by both parties and by any third parties required by the provider (veterinarian, transport, insurer).
