Recitals

WHEREAS, Client is the owner or lawful custodian of the pet(s) described in this Agreement and desires to engage Provider to perform pet care services described below; and

WHEREAS, Provider represents that Provider is qualified and able to perform such services and has the necessary experience, equipment, and personnel to care for Client's pet(s) in accordance with the terms and conditions set forth herein.

NOW, THEREFORE, in consideration of the mutual promises and agreements contained herein, the parties agree as follows.

Provider shall perform pet care services for the pet identified above. Services may include feeding, walking, grooming basic care, administering medication as instructed, overnight care, and transportation to veterinary appointments only as authorized below. Late Fee: % of outstanding balance, assessed after days.

Term and Termination

This Agreement commences on and terminates on unless earlier terminated in accordance with this section.

Either party may terminate this Agreement for convenience by delivering written notice at least days prior to the intended termination date. Provider may terminate immediately if Client fails to pay fees when due or if Client's pet presents a safety risk.

Cancellation Fee: may apply for cancellations made within hours of scheduled services.

Emergency Veterinary Care

In the event of illness or injury to the pet, Provider is authorized to seek veterinary care and to transport the pet to a veterinarian. Provider will attempt to contact Client and Client's emergency contact prior to obtaining non-routine treatment if time permits. Client authorizes Provider to authorize treatment up to $ without prior verbal authorization. Expenses for veterinary care and related costs are the sole responsibility of Client and must be paid directly to the veterinarian by Client or reimbursed to Provider within days of notice.

Liability, Indemnification & Owner Responsibilities

Client represents that the pet has no known aggressive tendencies and is up to date on vaccinations as indicated above. Client shall be responsible for informing Provider of any behavior issues, escapes, or special handling requirements. Provider will exercise reasonable care in performing services but is not acting as a veterinarian and does not guarantee results.

To the fullest extent permitted by law, Client shall indemnify and hold harmless Provider from and against any claims, liabilities, losses, damages, costs and expenses (including reasonable attorney's fees) arising out of Client's breach of this Agreement, Client's negligence, or the pet's behavior. Provider's liability for direct damages arising from Provider's negligence shall be limited to the total fees paid by Client under this Agreement for the applicable service period.

Confidentiality

Provider shall keep confidential all non-public information regarding Client and Client's pet obtained in the course of performing services and shall not disclose such information except as required by law or as necessary to provide services (including disclosure to a veterinarian or emergency responder).

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of without regard to conflict of law principles.

Entire Agreement

This Agreement constitutes the entire agreement between the parties concerning the subject matter hereof and supersedes all prior and contemporaneous agreements, representations, and understandings. Any amendment or modification of this Agreement must be in writing and signed by both parties.

Any notice required or permitted under this Agreement shall be in writing and delivered to the addresses set forth below or to such other address as either party may designate in writing.
