What the Pet Care Dog Check In Is and when it’s used

A Pet Care Dog Check In is an intake and consent form used by pet sitters, dog walkers, boarding facilities, and veterinary clinics to record a dog’s health status, behavior, routines, emergency contacts, and owner authorizations at the time of arrival. It collects vaccination data, medication instructions, feeding and exercise schedules, and any liability or care-release acknowledgements. The form can be completed on paper or electronically and typically includes a signature block where owners confirm accuracy and grant permission for specified care actions or emergency veterinary treatment.

Why a formal check-in matters for pet care providers

A completed check-in reduces risk, documents client instructions, enables consistent care, and provides evidence of consent for treatments or emergency actions. It also streamlines handoffs between staff and supports compliance with facility policies and insurer requirements.

Why a formal check-in matters for pet care providers

Who completes and signs this check-in

Typical users include staff and pet owners who exchange custody at the start of service. Fillers record facts, consent, and authorizations.

  • Pet owners who deliver a dog for boarding, grooming, or daycare and must provide health and contact details.
  • Pet care staff who intake the dog, note condition on arrival, and record special care instructions for handlers.
  • Veterinary or emergency clinicians when owner authorization is required for non-routine care or transport to a clinic.

The same completed form serves as the facility record, a client receipt, and a source document if disputes or medical follow-up are required.

Step-by-step: completing the Pet Care Dog Check In

Follow this sequence to collect clean, verifiable information and a valid signature at check-in.

  • 01
Gather ID: Ask the owner for full name and contact details.
  • 02
Record health: Enter vaccinations, medications, allergies, and veterinarian contact.
  • 03
Capture instructions: Note feeding times, leashing rules, and behavior concerns.
  • 04
Obtain signature: Have owner sign and date to confirm consent and accuracy.

Configuring an online check-in workflow

Set up fields, notifications, and routing for the digital version to match your on-site process and staff responsibilities.

Field Configuration
Owner Contact Required; phone and email validation
Vaccination Upload Optional file upload; PDF/JPEG accepted
Emergency Consent Checkbox plus signature required

Where completed check-ins should go and how they’re used

Define submission destinations and retention steps so staff and owners know who sees the record and when.

  • Local File: Paper copy kept with the dog’s intake folder
  • Digital Record: Saved to facility management system or secure cloud
  • Staff Access: Assigned caregivers view instructions and logs
  • Owner Receipt: Provide signed copy to owner by email or print

Technical considerations for digital check-in and eSign

Ensure the chosen provider offers encryption in transit and at rest and, if handling health data, a Business Associate Agreement for HIPAA compliance.

  • File Formats: PDF and DOCX accepted
  • Authentication: Email, SMS code, or stronger options
  • Integrations: Works with booking or CRM systems

  • Using generic forms without behavior or medical specifics increases the chance of missed details and preventable incidents.
