PET CARE PET RIDER AGREEMENT

This Pet Care Pet Rider Agreement (the Rider) supplements and amends the existing Pet Care Service Agreement between the parties identified below. Client Name: and Service Provider Name: hereby agree as of Effective Date: to the terms set forth in this Rider.

RECITALS

WHEREAS, the Service Provider performs pet care services including but not limited to feeding, walking, administering medication, and temporary boarding; and

WHEREAS, the Client owns or is the primary caretaker of the pet(s) described in this Rider and desires to engage the Service Provider to provide the additional or modified services set forth herein; and

WHEREAS, the parties intend for this Rider to be incorporated into and made a part of the underlying Pet Care Service Agreement and to control in the event of any conflict.

PAYMENT TERMS

Client shall pay the Service Provider the fees for pet care services as set forth below. All fees are due in accordance with the schedule and are non-refundable except as expressly provided.

Service Provider may suspend services for nonpayment following written notice if payment is not received within the time stated in the Payment Schedule. Client remains liable for all unpaid fees and collection costs.

TERM AND TERMINATION

This Rider commences on Start Date: and continues until End Date: unless earlier terminated in accordance with this Rider.

Either party may terminate this Rider without cause upon written notice to the other party at least days prior to the effective termination date. Termination for cause may be immediate for material breach including but not limited to nonpayment, abandonment, or animal cruelty.

CONFIDENTIALITY

The parties agree that any non-public information regarding the pet, Client's address, security codes, keys, or other sensitive details disclosed in the course of performance shall be kept confidential and used solely for the performance of services under this Rider. Service Provider shall not disclose such information to third parties except as required to provide veterinary or emergency care or as required by law.

AUTHORIZATION FOR MEDICAL CARE

Client authorizes the Service Provider to seek veterinary care for the pet(s) in the event of illness or injury if Client or Client's emergency contact cannot be reached within a reasonable time. Client shall be responsible for all reasonable veterinary expenses incurred. Client authorizes emergency treatment up to $ without further consent; for amounts above such limit Service Provider will make reasonable efforts to contact Client prior to treatment unless delay would endanger the pet.

CANCELLATION AND RESCHEDULING

Cancellations or rescheduling by Client must be given at least days prior to the scheduled service time to avoid a cancellation fee. Cancellation fee: $ . Repeated late cancellations may result in termination of services.

LIABILITY AND INDEMNIFICATION

Client acknowledges that pet care carries inherent risks. To the fullest extent permitted by law, Client releases and holds harmless the Service Provider, its employees and agents from liability for damages arising from the pet's behavior, escape, or injury, except where direct gross negligence or willful misconduct by the Service Provider is proven. Client agrees to indemnify and defend Service Provider from any claims, costs, or damages related to the pet's actions while under Service Provider's care.

GOVERNING LAW

This Rider shall be governed by and construed in accordance with the laws of the State where the services are performed. Any dispute arising under this Rider shall be subject to the exclusive jurisdiction of the courts of that State.

ENTIRE AGREEMENT

This Rider, together with the underlying Pet Care Service Agreement, constitutes the entire agreement between the parties regarding the subject matter herein and supersedes all prior agreements, understandings, and communications, whether written or oral. Any amendments to this Rider must be in writing and signed by both parties.
