WHEREAS, Owner engages Provider to perform specified pet care services for Owner's puppy identified below; and

WHEREAS, Provider warrants that Provider has the experience, staff, and facilities necessary to provide care in accordance with the terms set forth in this document; and

WHEREAS, the parties desire to document the scope of services, payment terms, and a checklist of routine care tasks and emergency instructions for the puppy.

Provider will perform routine care and supervision as specified below. Provider shall exercise reasonable care and act in the best interest of the puppy consistent with accepted pet care practices.

Payment Terms

Owner agrees to compensate Provider as set forth below. Unless otherwise agreed in writing, Owner is responsible for payment for services performed and for any emergency veterinary expenses incurred on Owner's behalf.

Term and Termination

This Agreement commences on the Start Date and continues until the End Date unless earlier terminated in accordance with this section.

Either party may terminate for material breach if the breaching party fails to cure within the notice period. Provider may immediately terminate or suspend services if the puppy's behavior poses an immediate risk to people or other animals, or if Owner fails to timely pay undisputed fees.

Confidentiality

Each party shall keep confidential and not disclose to third parties any nonpublic information obtained from the other party in connection with this Agreement, including medical, behavioral, and contact information regarding the puppy, except as required to provide the services or as required by law. This obligation survives termination for a period of three (3) years.

Governing Law

This Agreement is governed by and construed in accordance with the laws of the state indicated below without regard to conflict-of-law principles.

Entire Agreement

This document, together with any attachments or written amendments signed by both parties, constitutes the entire agreement between Owner and Provider with respect to the subject matter and supersedes all prior agreements, understandings, and communications, whether written or oral.
