This document combines a puppy care intake checklist and a binding service agreement between the Pet Care Provider and the Client. The Provider and Client agree to the terms set forth below and acknowledge the accuracy of the puppy information and authorization selections contained in this packet.

Recitals

WHEREAS, Provider is engaged in the business of providing pet care, supervision, and related services; and

WHEREAS, Client seeks to retain Provider to provide pet care services for the puppy identified in this packet, and Provider agrees to provide such services pursuant to the terms and conditions set forth herein;

WHEREAS, the parties agree that the effective date of services is unless otherwise agreed in writing.

Scope of Work

Provider shall perform basic puppy care services as requested by Client and agreed in writing, including feeding, exercise, supervised socialization, basic grooming, and administration of medications. Specific tasks, limitations, and special instructions must be set forth below.

Payment Terms

Client agrees to pay Provider the fees described below. All fees are earned upon performance of the services and are due according to the schedule selected.

Late payments will incur a late fee of the greater of the flat amount specified below or a percentage of the past due balance assessed per month.

Term and Termination

This Agreement commences on and, unless earlier terminated as provided herein, continues until .

Either party may terminate this Agreement for any reason upon providing the other party with written notice delivered at least days prior to the effective termination date. Provider may terminate immediately for nonpayment or if the puppy presents a danger to staff or other animals.

Confidentiality

Each party agrees to hold in confidence any nonpublic business, medical, or personal information obtained in connection with performance under this Agreement and not to disclose such information except as required by law or with the other party's prior written consent. This obligation survives termination of this Agreement.

Governing Law and Entire Agreement

This Agreement shall be governed by and construed in accordance with the laws of the state of without regard to choice-of-law principles. This Agreement, together with any written attachments executed by the parties, constitutes the entire agreement between the parties and supersedes all prior agreements and understandings relating to the subject matter hereof.

Emergency Authorization and Release
