What the Pet Services Document Is and why it matters

A Pet Services Document is a written agreement that defines the terms under which a pet care provider (groomer, boarding facility, daycare, sitter) will care for an animal. It typically records owner and pet details, services requested, schedule, fees, health and vaccination declarations, emergency veterinary authorization, liability limits, and cancellation terms. The document serves to set expectations, allocate risk, and provide contact and payment instructions. When signed by the pet owner and an authorized representative of the provider, it becomes a binding contract enforceable under state contract law and subject to electronic signature rules like ESIGN and UETA.

Why a clear Pet Services Document protects both parties

A clear Pet Services Document reduces disputes by documenting services, fees, vaccination requirements, and emergency care instructions. It protects both parties by allocating liability and setting cancellation and payment terms, and supports regulatory or insurance requests when proof of consent or care history is needed.

Why a clear Pet Services Document protects both parties

Who commonly completes and signs this document

Pet owners, groomers, kennels, sitters, and veterinary clinics commonly use the Pet Services Document to formalize care arrangements and responsibilities.

  • Pet owners: provide contact, payment, vaccination records, and emergency authorization.
  • Service providers: boarding facilities, groomers, and sitters responsible for care and recordkeeping.
  • Third parties: emergency vets, transporters, or authorized pickup persons listed in the agreement.

Tailor the form to the provider type and retain a signed copy for both the owner and the provider for recordkeeping and dispute resolution.

Essential sections to include in a professional Pet Services Document

Core sections of a professional Pet Services Document define parties, services, health requirements, payment terms, liability, and emergency procedures and contact information.

Service Description

Health & Vaccination

Require current vaccination records (including rabies where applicable), declare known illnesses, describe required flea/tick prevention, and state whether boosters or vet checks are needed before care begins.

Emergency Care

Obtain authorization for veterinary treatment, designate preferred vet and emergency contact, outline maximum authorization amount, and specify when provider must seek owner consent before non-emergency procedures.

Liability & Indemnity

Clearly state limits of provider liability, owner obligations for disclosed behavior issues, insurance responsibilities, and indemnification for damages caused by the animal during care, including third-party claims.

  • Using vague service descriptions like 'routine care' without specifics prompts disputes over included tasks and additional charges.
  • Incorrect dates or time windows for pickup/drop-off result in extra boarding fees or missed service slots and client dissatisfaction.

Real-world examples of how providers use the Pet Services Document

Practical examples showing how Pet Services Documents support daily operations at common provider types like kennels and mobile groomers.

Kennel Boarding

A community boarding kennel uses the Pet Services Document to record owner contact, pet medical history, service dates, and authorized caregivers before boarding begins.

  • This protects the facility and clarifies expectations.
  • When combined with photograph identification and updated vaccination uploads, the document reduces pickup errors, speeds incident response, and provides a clear basis for any damage or medical claims; retain records per your internal policy for potential inspections.

Mobile Groomer

A mobile groomer requires owners to sign a service agreement before arrival, specifying on-site procedures, power access needs, and whether sedative or medicated shampoos are permitted.

  • This prevents misunderstandings during home visits.
  • Including emergency contact and veterinary authorization allows the groomer to act if a medical issue arises during grooming; explicit billing and cancellation terms minimize disputes over time overruns or failed appointments.
