What the Pet Sitter Questionnaire Is and When to Use It

A Pet Sitter Questionnaire is a structured form used by pet owners to provide a sitter with essential information about an animal's care, routines, health, emergency contacts, and house rules. It centralizes feeding schedules, medication instructions, behavioral notes, veterinary contacts, and access details so a sitter can follow consistent care. The form reduces miscommunication during short-term visits, extended boarding, or house-sitting assignments. Completed questionnaires are commonly stored with the sitter’s records, handed to veterinary clinics in emergencies, and increasingly executed electronically using eSignature solutions subject to ESIGN and UETA frameworks.

Why a Pet Sitter Questionnaire Matters

The questionnaire minimizes risk and confusion by documenting critical care instructions and contacts, creating a single reference for sitters and caregivers.

Why a Pet Sitter Questionnaire Matters

Who Typically Completes and Relies on This Questionnaire

Common users include pet owners, professional sitters, boarding facilities, and veterinary clinics who need clear, retrievable care instructions.

  • Pet owners who travel or work long hours and need reliable written instructions for daily care.
  • Professional pet sitters and pet-sitting agencies that require standardized intake information before accepting assignments.
  • Veterinary staff or emergency clinics that receive the animal and need up-to-date medical and contact details.

Keeping the questionnaire current ensures any new sitter can deliver consistent care and that emergency responders have accurate information.

Privacy Breach: Exposure of personal data

Practical Tips for Accurate and Efficient Completion

Apply these practices to reduce errors, improve clarity, and streamline transfers between caregivers and clinics.

Use Clear, Unambiguous Language
Write instructions in plain language, specify exact quantities and times for medications, and avoid shorthand that sitters or vets may misread.
Keep a Medical Summary Sheet
Include vaccination status, chronic conditions, and last vet visit date so emergency clinics have critical information immediately.
Set Financial Limits
Specify the maximum emergency spend the sitter is authorized to approve to avoid disputes over vet bills or treatment decisions.
Test Digital Workflows
Run the eSignature process once before travel to ensure recipients receive links, authentication works, and signed copies download correctly.

Real-World Use Cases

Two concise examples show how different owners and providers use the questionnaire to reduce risk and improve care continuity.

Family Vacation

A frequent traveler prepares a detailed questionnaire with medication schedule and emergency funds authorization

  • Sitter receives digital copy and signature link for acceptance
  • When the dog developed an infection, the sitter followed instructions and the vet billed the owner directly, avoiding treatment delays and dispute.

Pet-Sitting Agency

A local agency requires a completed questionnaire before booking for new clients

  • The agency stores signed forms in a secure portal and assigns sitters with matching experience
  • Standardized forms reduced on-site errors and allowed rapid handoff between sitters during a multi-week house-sit.

Who Signs and What Authority They Hold

Pet Owner — Primary

The owner signs to confirm instructions and to authorize emergency medical care up to specified limits; the signature demonstrates intent and attribution required under electronic signature laws.

Pet Sitter — Assignee

The sitter signs to acknowledge receipt and acceptance of responsibilities and any financial or treatment limits; signature documents consent and reduces later disputes.

Amend:

Create a new version with updated effective date
02

Notify:

Send revised copy to sitter and vet
