Make sure to attach a copy of the “SATOP Notice of Offender Assignment Form ” (A3) with this petition before filing. 3. The filing of this petition is a civil action for which you will be expected to pay a c ivil cost deposit at the time of filing. 4. It is best if you use the name of a service provider (i.e., the business or company) for the r espondent instead of the individual who administered the test and held th e personal interview with you. If you use the name of the individual, there may be times when the person no longer works for the provider and the petit ion cannot be served upon them. The court may dismiss the case if service cannot be made on the party lis ted as the respondent. 5.
