What the PlayCare Services Contract Covers

The PlayCare Services Contract is a written agreement that sets the terms between a childcare provider and the family or guardian of a child receiving care. It documents services, schedules, fees, responsibilities, emergency procedures, authorization for medical care, and termination terms to reduce misunderstandings and support regulatory compliance.

Why a Clear PlayCare Services Contract Matters

A well-drafted PlayCare Services Contract clarifies expectations, reduces disputes, and provides evidence for licensing or insurance reviews. It also establishes payment terms and liability allocation to protect both providers and families.

Why a Clear PlayCare Services Contract Matters

Who Typically Completes a PlayCare Services Contract

The contract is completed jointly by the childcare provider and a parent or legal guardian before care begins.

  • Parents and Guardians sign to authorize care, confirm emergency contacts, and accept fee and pickup policies.
  • Center or In-Home Providers complete operational fields, schedule blocks, and safety procedures to meet licensing requirements.
  • Agency Administrators or Program Directors finalize fee schedules, subsidy details, and provider credentials when applicable.

Both parties should retain a signed copy and review renewal or amendment terms when schedules or fees change.

Primary Signers and Their Roles

Parent / Legal Guardian

Signs to authorize the child's enrollment, emergency medical care, and pickup permissions. Responsible for fees and for notifying the provider of changes to health, custody, or contact information. Their signature creates consent for daily care and specified releases.

Provider Representative

An owner, director, or authorized staff member signs on behalf of the childcare entity to confirm services, schedules, staff ratios, policies, and liability limits. Their signature binds the provider to deliver care consistent with licensing and the contract terms.
