This Player Participation Form and Agreement (the Agreement) is entered into by and between the organization and the individual player identified below. The parties agree to the terms and conditions set forth herein which govern the player's participation in programs, practices, competitions, and related activities organized by the organization.

RECITALS AND SCOPE

The player will participate in organized activities including practices, training sessions, matches, travel to and from events, and associated meetings. The player shall comply with all rules, codes of conduct, safety protocols, and directives issued by the organization.

PAYMENT TERMS

In consideration for participation, the player shall pay the organization the fees set forth below. Failure to timely pay fees constitutes a material breach and may result in suspension of participation until cure or termination.

Late payments shall incur a late fee of on overdue balances and interest at the lawful maximum rate where permitted.

Either party may terminate this Agreement for material breach if the breach is not cured within days after receipt of written notice. The organization may suspend participation immediately for safety or disciplinary reasons.

CONFIDENTIALITY

The player shall not disclose confidential business information of the organization including proprietary training methods, non-public scouting reports, strategic planning, financial terms, or personal information of other participants. Confidential information excludes information in the public domain or independently developed by the player without use of the organization's confidential information.

WAIVER, ASSUMPTION OF RISK AND INDEMNITY

The player acknowledges that participation involves inherent risks including physical injury, illness, and property loss. The player voluntarily assumes all such risks. To the fullest extent permitted by law, the player releases, waives and discharges the organization, its officers, agents, volunteers and employees from claims, liabilities, losses or damages arising out of or in connection with participation, except for willful misconduct or gross negligence.

The player agrees to indemnify and hold the organization harmless from and against any claims, demands, losses or damages (including reasonable attorneys' fees) arising out of the player's acts or omissions while participating.

PHOTO AND MEDIA RELEASE

The player grants the organization the right to photograph, record and use the player's name, image, likeness and voice in connection with promotional, training, or educational materials, in any media, without further compensation, subject to the organization's policies and applicable law.

GOVERNING LAW; ENTIRE AGREEMENT

This Agreement is governed by the laws of the state or jurisdiction identified by the organization. Any dispute arising under this Agreement shall be resolved in the courts of that jurisdiction. This Agreement constitutes the entire agreement between the parties with respect to participation and supersedes all prior agreements, oral or written, relating to the subject matter hereof.
