Recitals

WHEREAS, Service Provider: is duly licensed and equipped to perform plumbing repair and maintenance services; and

WHEREAS, Client: owns or is authorized to arrange repairs at the Service Address identified below and desires certain plumbing repair services; and

NOW, THEREFORE, in consideration of the mutual covenants set forth below, the parties agree to the terms and conditions of this Plumbing Service Repair Form.

Payment Terms

The Client agrees to pay the Service Provider the sums described below for the Scope of Work. All sums are due in U.S. dollars unless otherwise agreed in writing.

Term and Termination

This Agreement commences on Start Date: and continues until completion of the Scope of Work or until End Date: , unless earlier terminated in accordance with this section.

Either party may terminate for material breach if the breaching party fails to cure within Notice Period: days after receipt of written notice. Service Provider may suspend work if Client fails to timely pay any undisputed amounts.

Warranty & Limitations

Service Provider warrants that work performed by its technicians will be free from defects in workmanship for Warranty Period: months from date of completion. Warranty excludes damage due to misuse, modifications, pre-existing conditions, or failure to maintain systems. Parts supplied carry the manufacturer's warranty only.

Confidentiality

Each party agrees to keep confidential any non-public business, technical, or financial information disclosed in connection with this Agreement. Confidential information does not include information that is or becomes publicly known through no breach of this Agreement or is required to be disclosed by law or valid process.

Liability and Indemnity

Service Provider's liability for claims arising out of this Agreement shall be limited to direct damages not to exceed the total amount paid by Client to Service Provider under this Agreement for the specific repair giving rise to the claim. Neither party shall be liable for consequential, punitive or incidental damages. Client shall indemnify and hold Service Provider harmless from claims arising out of Client's negligence, misuse, or failure to obtain necessary permits where required.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the state identified by the Service Provider's principal place of business, without regard to its conflict of laws principles.

Entire Agreement

This document, together with any written change orders or attachments signed by both parties, constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior proposals, negotiations and representations, whether oral or written. Any modification must be in writing and signed by both parties.
